Suggest Remedy For Severe Constipation In A Child

Question: Hello Doctor, my kid is 2.6 years old now. He has constipation problem since he was 1.2 years old. We met several doctors and very frustrated as all doctors just change the medicine like - earlier we started with Creamafinn than XXXXXXX Q again Creamafinn + XXXXXXX Q and now Evict. Since 1.2 years of age we are providing him home made XXXXXXX Fruits, Upma, Home made all vegs, chapati, rice, fiber foods, enough water intake etc. Now problem is since he is taking medicine last almost 1.5 years if he does not take day or two, its very difficult for him to pass the soft stool.
We also did whole Abdominal, Thyroid, Vit D etc tests. He got Vit D is low and rest tests are normal. Existing doctor said its normal altogether but changed the medicine.
We are scared if all the fine why he is not having normal stool without medicine.
We are also scared for the long term of using these medicines. Please advice.

XXXXXX> He passed motion on day one. I think yes Meconium he had on first day.

2. Since how long is the kid constipated?
XXXXXX> Since, he was 1.2 years old.
In the beginning he used to force and we did not notice the same as there was slightly hard stool with no blood. But after around 1 month he had bleeding in his stool. Since than he has this problem. Now since than we are providing him Creamaffin, XXXXXXX Q and now Evict. When we do the doctor changed, our medicine changed and his stool gets soft. But now he is taking medicine from more than year and we try to reduce, he has the problem again.

3. Does the kid have any bleeding along with hard stools?
XXXXX> yes, forgot to write earlier. Earlier it was little but even by consuming medicine, he has bleeding once in every week or in 10 days though his stool is normal, if he takes medicine. If we try to reduce the medicine for a day or 2, hard stool + blood.

4. How much milk does the kid consume per day?
XXXXXX> As recommended by docs, his milk is stopped since 6 months ago. We used to give him Butter milk, home made curd, sometimes paneer etc instead of milk.

5. Does the kid eat fruits and vegetables (fibre diet) appropriately?
XXXXX> He loves all fruits so we give him all fruits. Doc suggested not to give him Banana, so we have skipped Banana from last 2 months. In vegetables we give him XXXXXXX Khichri, Upma, Home made Idli, Normal Oats, Rice, potato, tomato, palak (spinach), moong dal palak vegetables, chana dal + lauki (dudhi/Bottle Gourd), home made soups, juice of tomato, carrot, spinach etc, In sweets my wife used to prepare at home some of the suits. Even we have changed our diets by taking all these above. Hope we are giving all the necessary foods.
As per doc recommendation no junk foods (biscuits, chocolates, chips, toffees, bread, namkeen etc) since last 3 to 4 months.

6. Is there any family history of constipation?
XXXXXX> During pregnancy my wife had a around 1 month of constipation problem. Thats all. No one has problem for constipation at home.

7. How much water does he consume per day?
XXXXX> He used to take around 1L (not always). We have his separate glass, 3 to 4 glasses (250ml) apporx.

8. Earlier he used to take milk using bottle but its been stopped since a year ago. So, no bottle feeding.

1. Natural methods are the best to relieve constipation.
2. Constipation is a risk factor for UTI
3. Maximum milk consumption per day should not exceed 300ml
4. Minimum 3-4 cups of fruits and vegetables to be consumed per day. Best will be papaya daily morning and evening.
5. Toilet training - that is - sitting in XXXXXXX type of lavatory daily at the same time will help a lot.
6. Minimum 2 litres of water per day.

O suggest you do an investigation called Barium enema after consulting your pediatrician to rule out a congenital condition called Hirschsprungs disease.

I think my problem is not solved as we are maintaining all the points mentioned by you (off-course we are giving papaya, musk melon etc). Also, as mentioned by other doctor Milk is stopped as of now. Why have I approached you is when we go to doctor they don't listen the complete history and used to change the medicine that's why I am very worried - what's wrong with my son. Please assist.

One of our doctor also suggested the Barium enema test (surgeon). But we were scared as our kid was < than 2 years at that time.

Need to understand the below from you.

1- Basic question is - if we are giving him Evict / XXXXXXX Q / Creamafinn, his stool is soft but reducing the dose results the harden of stool again. Why ?

2- Please can you elaborate point 2 ? Constipation is a risk factor for UTI.

3- Also, as medicine like Evict is going on and earlier XXXXXXX Q and Creamaffin etc was given. Will that be harmful for long run and we shall discontinue and rush for the Barium enema test now ? Is the only option for us as its almost more then a year ?

Hi,...coming to your queries directly...

1. After stopping medicines stool is getting hard because once the effect of medicine is going off - because of low water content in the stool the stool is becoming hard. The solution for this is to give even more water so that the large intestine will not absorb water from the stool and they stool remains soft so that it will not create a fear of painful defecation for the kid which is the root cause for habitual constipation like this.

2. Constipation is definitely a risk factor for urinary tract infection. This may occur on long-term but you need to be more careful if the kid with constipation like this develops burning sensation while passing urine or when he gets high grade fever with chills and rigors. These are the symptoms of urinary tract infection.

3. Any on long term medications can cause some complications because it is not a natural way to relieve constipation. But this medications which are used for constipation are in other way may cause diarrhoea. If that happens the kid might lose some Minerals and electrolytes in the stool which might cause some complications. But this can happen only with profuse diarrhoea. So do not worry about it right now.

3. If I were your pediatrician I would definitely suggest you to get the investigation that I have suggested in our previous interaction. I am not telling that it is an emergency but at the same time the kid definitely needs this investigation to rule out the congenital condition that I have elucidated in my last interaction with you.
