Suggest Remedy For Persistent Pain During Bowel Movement

Question: Hello Doctor,
4 days back i had pain while cleaning bowel due to hard stool.butter and bread diet with a bit of fibre improved the situation and pain disappeared with.Later a spicy diet brought in the same problem again 2 days back with pain while cleaning bowel and also persistent pain while sitting or lifting heavy things since 2 days.Fibrous food has loosened stool but pain is persistent.Also no blood in stool or anywhere around anus.Please help to know what i'm going through.

Hello,

it sounds like hemorrhoids. I don't know if you've checked the anus visually and if "nothing around the anus" means that there is no thrombosed hemorrhoid for example. If you haven't checked, please do so with the help of a mirror or you can ask someone to do it for you. A thrombosed hemorrhoid looks like a dark nut-sized (or a little larger) mass outside the anus. If you have no mass there, then perhaps an anal fissure causes the discomfort. That would be difficult for you to check. You'd need to visit your doctor for that.

Persisting with diet is the right way to go (enough fiber content). Try to drink enough fluids and follow a regular schedule for toilet visits. The bowels love to be on schedule! Go to the toilet at the same time every day and try to move your bowels even if you don't feel like it. Don't strain too much at this time though because if it's a hemorrhoid it may get worse. Let it cool off a bit first.
Finally being physically active helps with bowel motility and many other things healthwise, so you'd better make sure that you're not sedentary.

Thank you very much Doctor.
We checked it here.There is nail tip size whitish pink protrusion on opening and seeing inside the anus hole;it doesn't feel on touching using finger.There is one more small dark brown swelling behind the same place in the area 2mm by 4mm area,less than 1 mm thickness.Though not thick can be just felt.
There is a bit of wet felt while trying to check,though had cleaned dryly earlier and smelt bad,but not like stool.It is not blood also.
Do you suggest immediate,emergency action,to avoid further problem,or is it ok to wait for a day to check out bowel reaction to changed healthier food habit?
Also appointment of minimum 4 days or so is required to visit family physician,but if it is much urgent call,then we will have to visit Emergency healthcare,so please suggest.

The appearance and symptoms could point to a perianal abscess (the whitish pink appearance, wet feeling and bad smell, pain when sitting or defecating), which I haven't mentioned because it's not a common situation. If it's indeed an abscess then drainage would be - most likely - required. You'd better visit a doctor (a general surgeon would be a good choice of a specialist) as soon as possible to clarify this issue.
