Suggest Remedy For Paranoid Mood Disorder In A Schizophrenic Person

Question: Respected Doctor,

This is XXXX. I am being treated for Paranoid Scheizopreniac with tablets JoyKem and Divalpro XR. I had this episode almost 6 years back but symptoms came to control and I stopped medication. Currently for past 1 year I have this symptoms again and started consulting pyschiatrits. For past 1 year, I felt my actions,phone calls,e-mails etc are being monitored. I thought/perceived people around me in office environment/public were giving some sort of signals that relates to any film and make me connected to it. Initially, I thought it was a way for divine intervention/blessings and left it like that.I am being very religious thought that people wanted to make me happy so that they receive divine blessings. I am not sure whether this is my perception or real. Couple of months back an incident affected me so much that I got deeply connected to the affected person and started supporting a person very much within my family circle and no way the person is related to me. I was also irrational in my thoughts. The incident made be so sad. This created mood disorders. This left me to behave not so good in office. Now, I started fearing(Paranoid) for everything. For example, when I read newspapers,news etc.... I don't get comfort feeling and creates some sort of fear. I think about the events happened couple of months back and that creates more fear about the way I spoke and behaved in office. I really wonder whether I caught up with delusions of monitoring my actions,phone calls, e-mails etc or those where real thing.Can I please continue to use JoyKem and DivalPro XR or any advanced treatment is required ?

Respected Doctor,

1)Initially, I was taking Risnia MD3.0 now that has been replaced with joykem 100 along with Divalpro XR 250. I don't have any side effects. But often gets paranoid.

2) I have been taking treatment from Psychiatrist often for this.

3) I have my Father,Mother and Wife but with no kids. Sometimes this is causing some sort of fear for future.

4) I don't smoke/drink.

Thanks for writing back.

From your description and the history you have provided, it appears to me that your delusions might be back. And you have superimposed mood symptoms.

The drugs you are taking are absolutely fine. But I think the drugs are under-dosed.

Therapeutic dose of Joykem (amisulpiride) for schizophrenia is 400-800 mg depending on the response of symptoms.

Divalproex is dosed according to body-weight. The therapeutic dose range in between 20-30 mg per kg.

If adequate dose is not given, the symptoms will keep re-appearing.

Its wonderful that you have not developed any side-effects from these drugs

In my opinion, you can continue with these drugs.

But discuss above issues with your treating psychiatrist and make any dose changes under your psychiatrist's supervision.
