Brief Answer:
Let us investigate for the exact cause first

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Female/75 - consulted Doctor - tried injections, spinal surgery, therapy and stretch therapy - spinal stenosis - pain in thighs - big knots in both thighs - wants to know what can be done to decrease lumps/pain...

Since you have already been treated with spinal surgery and other therapies as you have mentioned, I would like to think of hte reasons for your problems.
The Big knots have to be investigated for its origin:
Ultrasound and FNAC - fine needle aspiration cytology may help.
Once the cause of the knots are known, it is easy to plan further management.

Till then you can have anti-inflammatory medicines with painkiller properties like Ibuprofen if it suits you.
Give rest to the part, means avoid too much strain on the thigh.

I hope this answer helps you, please feel free to ask for further relevant queries if you feel that there is a gap of communication.
