Brief Answer:
Seems to be due to infection of the genitourinary system.

Detailed Answer:
Thanks for the consultation at my virtual clinic HCM!
I have carefully worked through your case, and can well realize your health worries. Being your physician, I will take care all of your medical concerns today and will put my best efforts to get you out of this situation.

Clarification:-
Masturbation or a safe and protected sexual intercourse rarely cause an issue like this.

Probability:-
You have got some infection of the Genitourinary tract and due to that there is a feeling of pain in groin and testicles specifically related to ejaculation. Most probably Epididymis infection (Epididymitis) leads to this painful situation.

Medication Guide:-
If I was your treating physician I would have prescribed you with Azithromycin antibiotic class of medicine if not allergic to. Plus:
Take Ibuprofen for pain relief. If you feel pain even after you get Ibuprofen, you may take Fioricet with codeine by mouth.

Remedies:-
Limit physical activity when you have pain.
Ice packs will help when in pain. Also elevation of scrotum with scrotal support will help relieve pain.
Sitz Bath (Take a tub full of Lukewarm water with some salt in it. Sit 3-4 times a day in that water dipping the genitals in that).
Plenty of water and try to void urine after sexual activity so that there is flushing of the tract with urination.

Hope I answered all of your questions. Let me know if you have further clinical concerns or doubts. I will be more than happy to get back to you with expert opinion. Please don't forget to give Five Star rating to help me assist my patients in a better way; and, please don't forget to 'Close This Discussion'.
For any disease you have, a timely follow-up is very necessary regarding the treatment efficacy and to see if there is a need to change the treatment options. So keep a follow up to see how you doing (Always write down my name in the start of your questions at HCM & I will be with you in no time).
