Question: Hello,
I was prescribed citalopram 20mg like three weeks ago but I took just 7 pills because I was having very bad side effects like nauseas, insomnia, tingling in my arms and back very badly I thought it was affecting my nervous system so I decided to stop it, I went to the doctor and he told me to continue with it and gave me trazodone 50 mg for sleeping, I just started trazodone (2 days ago) it makes me feel relax and calm but I do not sleep at all, yesterday I took melatonin and I could sleep like 2 hours then I woke up and I took trazodone 50mg but I did not sleep at all, I am a new immigrant in this country but I think I do not have big depression just sad because I moved to here and no family, etc but I still smile and do things I like, my biggest problem is sleeping because I am not functioning, I am doing a lot of errors at work, my eyes are so irritated, red and I have headaches due to lack of sleep. Can you please recommend if I should continue with trazodone? is it normal to have the effects after days or why is not working for me because I do not sleep. Or should I continue with melatonin or what can I do to improve my sleep. Also I am taking metformin 500mgs I was just prescribed I am diabetic type 2. I do not know if the combination of these medicines can affect me but as I said before what I really need is to sleep, I am doing exercise almost every day around 45 minutes and yesterday I went to swim and I was exhausted and feeling sleepy but I slept just 2 hours and then took the trazodone but awake all night is second night I am awake with trazodone. Please help me to understand what going on? I was never before on medication for depression or sleeping problems.
Thank you
doctor

Some other options to help you

Detailed Answer:
Hello,
Thanks for choosing health care magic for posting your query.
I have gone through your question in detail and I can understand what you are going through.
It appears you are having a depression and sleep problem. For sleep problem you need to follow all the steps of good sleep hygiene.
1) Have some aerobic workouts during day preferably in the morning.
2) Have a light dinner.
3) Completely avoid coffee/tea/alcohol/energy drinks/coke/smoking.
4) Have a warm water bath before going to sleep.
5) Dont try very hard to go to sleep. If you are not sleepy then sit on a couch or sofa and do some reading and go to the bed only when you are sleepy.
6) Avoid watching tv or working on laptop/computer or bright screens at night.
7) Keep the lighting of your bedroom dim and make your bed every morning after you wake up so that you get a cozy bed to sleep at night.
8) Use your bedroom only for sleeping and resting and do not do any other work there.
9) Avoid afternoon nap for any reason.
Further if the melatonin or trazodone is not helping you then you may opt for zolpidem or eszopiclone. These are also safe and effective sleep medicines which you may take for 2-3 days until your sleep improves.
Regarding depression it can occur with regular stress and it can also occur biologically without any stress as well. You need proper treatment for the same. There are various effective therapies for depression like fluoxetine, venlafaxine, bupropion, mirtazapine or escitalopram and they are all very effective and mostly safe. You may also require some benzodiazepines like clonazepam or lorazepam for a short period like 2-3 weeks to reduce the anxiety and help with your sleep.
If you are averse to the drug therapies then there is also an option of cognitive behavioural therapy. This therapy is taken by either a psychologist or a psychiatrist in which the therapist identifies impaired cognitions (Thoughts) and try to correct them. They are 15-20 min weekly session and there should be around 8-10 sessions to help you with your illness.
Do meet your GP and discuss these options.
Hope I am able to answer your concerns.
If you have any further query, I would be glad to help you.
