Question: The glands on the foreskin of my penis have been swollen. It seems that under the foreskin my penis is irritated and soreI have been using over the counter ointments such as Neosporan, but the situation does not seem to be improving. What should I do?

Hello and welcome,

It sounds like the balanitis did not resolve with the clotrimazole -which is an anti fungal, nor with the Neosporin - which is an antibiotic for bacteria.

Most cases of balanitis are from fungal infections such as candida. You may need an anti fungal cream different from clotrimazole. A good anti fungal for this is ketaconazole. This is by prescription only.

If the skin is getting quite inflamed and swollen, in addition to a different anti fungal, you may need a topical corticosteroid cream to bring down the swelling.

So you may want to call the doctor on call for your doctor to get the ketaconazole and a steroid cream.

It may also be a good idea to go in to have the skin swabbed for a culture if the problem persists.

If you develop a situation where you can't retract the foreskin, then you will need to go in to an urgent care or ER promptly.
