Suggest Remedy For Increased Systolic BP

Question: Hi.
I am 64 y/o, healthy , normal weight , exercise moderately,
I am a pediatrician, working hard .
I ve noticed in a past month encrease systolic pressure , I always been normotensive 120 / 60 ,or less .puse
Between 55-65 ,
It's been getting worse in a past few days ,
Systolic went up today up to 188/75 , pulse 64 ,
Completely asymptomatic ,
Never taking melds for anything.
Can't recalled what might trigger such increase .
I use on occasions some homeopathic melds ,but not frequently
And Premarin cream on occasions .
Blood being sent today for complete work up of hypertension .
I feel fine , except , head feels full , warm ,
Any advise ?

Hello, I am Dr Mody and I would be addressing your concern.
I would be precise to help answer your query.
1) The warm feeling in the head is most probably due to increase in the systolic pressure.
2) If just renal cause by getting S. creatinine, urine routine , usg kub with renal artery doppler to rule out the most common secondary cause of hypertension in your age is ruled out we can safely consider it as essential hypertension
3) previously it was thought that systolic blood pressure doesn't have the same complications as the diastolic rise had and so in asymptomatic patients no treatment was offered , however enough data in last 10 years suggesting we should treat it to bring it less than 140 over time (160 initially).
4) you need not worry about the pulse as that's pretty much in normal range, you the borderline low pulse is reflex to higher blood pressure
Now important here is to establish whether you have hypertension or not.
A 24 hour ambulatory BP monitoring is the best way to know even if you are in pre hypertension stage.
By the time start with low salt in diet and considering you yourself are a XXXXXXX doctor wouldn't get into lifestyle modifications.
If the ambulatory monitor shows higher reading strongly consider an anti hypertensive.
A ace inhibitor /arb with a small dose of diuretic will be ideal.
I would comment on homeopathic medication as that is not my forte.

That was very informative answer .
My low pulse reading I have all my life .
I am on low sodium diet most of the time ,
I lead pretty healthy life style .
That what concerns me ,
My biggest risk factor is positive family history of hypertension.
And aging process taking place with all menopausal problem .

Mam, I would not say that the pulse are on lower side, rather they are on healthier side. It means your heart is well trained and can cope up with exertion with minimum tachycardia.
All athletes will have a lower pulse rate, maybe your healthy lifestyle has been a reason for lower pulse rate. Ideal pulse is between 50-60.
Yes family history and normal aging process (30% prevalence of hypertension more than 60 years) is itself strongest risk factors for hypertension.
So my logic here will be to address it if we confirm that the blood pressure is persistently in higher range. To prevent any complications in future due to higher blood pressure.
