Question: My mother is a diabetic as well a high BP patient (Age 56 years). Recently her sugar level has gone upto 380. We have been consulting many doctors from last 10 years. She is taking Galvus Met (50/500) and Zoryl - 1 for diabetic , earlier she was also taking Tonact TG but when we shifted to a new city she stopped taking the same. Can she continue Tonact TG and wheter the other diabetic medicines are right for her usage??
Also earlier she used to take Nodan H & Eritel AM for high BP but when we shifted to a new doctor in a new city, she started taking Amlopres-AT & Ecosprin-75. Is it right?? Which medicine she should take either the first set or the second set of medicnes???

Brief Answer:
Need further evaluation.

Detailed Answer:
Hi,

Thanks for your question.

After going through your question, I can understand your concern.

For her Diabetes which is uncontrolled. She may need Insulin along with Tab Galvus Met and Zoryl.

Tab Tonact TG contains Atorvastatin and Fenofibrat. She should continue Atorvastatin regarding dose of Atorvastatin and whether to continue Fenofibrat, it will depend on her Lipid Profile.

For Her blood pressure I may prefer Eritel AM and need for another antihypertensive like Nodal H depending upon her blood pressure levels. But her Kidney Function and presence of heart problem may change the decision.

If there is no heart problem or stroke or peripheral vascular disease, I would have started Ecosprin after 60 years of her age.

It is better to discuss your issues with diabetologist near by, so he can take appropriate decision depending on her clinical Examination and investigation.

Following investigation may play significant role in decision making -
1) Complete blood count.
2) HbA1c.
3) Kidney function test.
4) Urine for albumin creatinine ratio.
5) Lipid profile.
6) ECG and if required 2DEcho of heart.

Hope I have answered your questions.
I will be happy to answer your follow up questions, if any.
Otherwise you can close the question and rate the answer.

Thanks Sir for your prompt reply.

I would certainly get those test done and then come back to you again.

I also wanted to ask you about her diet plan. I had lot of doubt whether she should stop eating rice as it contains lot of carbohydrate?? What fruits she can take as all fruits has sugar in it?? Please suggest some the the right diet for her.

Does Amla (awala) juice or any other juice help her in controlling her diabetes. plz suggest.??

Does any kind of exercises or yoga help her health get better or at least keep her healthy not affect her??

Brief Answer:
Diet

Detailed Answer:
Hi Mr. XXXXXXX

Thanks for your followup question.

Usually diet depends on Patient's wight and BMI ( Body Mass Index)

Usually for average weight Diabetic XXXXXXX women 1600 kcal with 60 gram of proteins recommended.

You should consult near by dietitian who can give diet plan depending up on your mother's likes and dislikes.

Regarding fruits if there is no kidney problem,
- She can have citrus fruits as a whole (not fruit juices) like daily one orange / sweet lemon.
- She can have 1 to 2 small slices of pineapple, 1 whole small to medium sized apple, 1 bowl pieces of papaya.
- Fruits like XXXXXXX banana, chikoo, custard apple should be avoided for regular use. These types of fruits can be taken occasionally only for taste.
- She can have Amla which gives fibers and vitamin c and has antioxidant property.

Regarding rise she can have 1 small bowl of rise with lunch daily.
Rise with long grain which contain more fibers like basmati rise is better.

150 min per week of any type of moderate degree exercise is recommended.
She can do yoga, brisk walk.
Exercise pattern need to be decided depending upon her heart condition.

Hope this helps you.
