Question: my mother's thumb is swollen. she is already taking flucanozole med once in a week and the cream twice or thrice everyday. their is no pain but swelling is not going attached is the photograph pl, advice

Hello. Thank you for writing to us.

I am Dr. Kakkar (dermatologist). I have read your question and I have also reviewed the image.

She has chronic paronychia which is chronic swelling of the lateral and proximal nail folds due to candidal infection.
The infection is more common in females who are engaged in household work involving frequent and prolonged contact with water like wasing dishes and doing laundry. This prolonged immersion in water causes candida to invade through the nail cuticle and cause inflammation, pain and swelling of nail folds.
Fluconazole is an oral antifungal which helps to control candidal growth and therefore helps in chronic paronychia. However, unless and until she stops doing wet work she might continue to have recurrences as there will be repeated infections with candida and the swelling would become more persistent.
Therefore apart from oral antifungals and topical cream she must also avoid regular household work requiring prolonged hand immersion in water so that she does nt develop recurrences and the swelling would then go down with time.
