Brief Answer:
Infectious cause likely, needs workup

Detailed Answer:
Hello and Welcome

I appreciate your concern

Since the response to saline spray, netti wash and decongestants was not adequate, an infective cause should be looked for. This especially required with the history of chest tightness which you mentioned , got worse when you are in prone position. ECG, CBC, ESR, Chest x ray and pulmonary function tests can be advised after physical examination. ECG would be required to rule out a cardiac cause. As far as the treatment is concerned, Antibiotics, inhaled bronchodilators and steroids would be decided based on the results of the investigations and clinical signs. You may request a pulmonology consultation/referral via your GP.

Wishing you best of health

Thanks
