Posted on Tue, 31 Jan 2017
Question: Hi, I had a c section 12 weeks ago and ever since when I strain to go to the toilet I get a pain in my rectum.
I don't have any other pains only when I am straining.
What could this be?

Brief Answer:
As discussed in details below.

Detailed Answer:
Hi.
Thanks for your query, short and specific.
Noted the history carefully and understood your concerns.
The pain in rectum/anal area after C-section is usually due to fissure in ano.
Such pains come only on straining, passing stool.
Since it is 12 weeks, you have to get this correctly diagnosed first for which you need to get digital per-rectal examination and ano-proctoscopy, preferably by a General Surgeon.
Till then you can have the following treatment:
High fiber diet.
Avoid all the foods and beverages and stress-like conditions which gives constipation.
Laxative for softening the stool- never allow the dose too much to cause liquid stools.
Sitz bath- very important- just before passing a stool and after passing a stool.
Clean the area very clear of the fecal soiling that remains after cleaning; use cotton soaked in the same water to clean.
You can apply liquid povidone iodine locally. Apply nothing else.
5 day course of an antibiotic on prescription, if required.
Add probiotics and multivitamins.
Avoid stress and mental tensions at any cost; this is a very important factor.

I hope, this answer helps you. Please feel free to ask for further relevant queries if you feel that there is a gap of communication.

Thankyou for your response. I wonder if this possibly due to pelvic floor issues? The pain is more ache/bruise feeling as opposed to a sharp pain. It's concerning me Incase it's something serious or I may be over reacting.

May be

Detailed Answer:
Yes, there is a rare chance that the differential diagnosis is pelvic floor issues if the per-rectal examination and ano-proctoscopy is within normal range.
If there is a pain, there is a reason.
