Question: Hi, 12 days in, after a fall and a huge hematoma on my hand, what would you recommend for major discomfort, heat or cold? By the way, no fracture, and I'm 73 year old very healthy female.

Brief Answer:
I would use what gives you comfort, but add Tylenol

Detailed Answer:
Welcome to HCM: Please note that the anti-inflammatory response you would get from cold packs would have helped 12 days ago when you hurt yourself, at this point you are doing everything correctly. I would use whatever provides you relief.

I have patients who prefer cold over hot and vice versa. I would add Tylenol as this will provide actual pain relief that you are needing.

I hope this helps and thanks for your question.
