Question: I asked a question which was answered by Dr XXXXXXX XXXXXXX However I am not able to access The follow up response on my email. I would like an urgent response to my question please . This can be done by him or any gynae specialist urgently.

I read on your website that the way to induce a period is by taking primulot on day 15 of Cycle. (5 mg three times a Day For 5 days). I did this from day 15 for the duration but have not a period 6 days later (except for very very light brief spotting which disappears).

The reason for this is that I am going on Pilgrimage from the 15th September. I was due to get my period around that time and wanted to bring it forward. The main pilgrimage days are from the 3rd to 7th October. Dr XXXXXXX Indicates that I will get my period on day 15 of last dose. And another breakthrough bleed 15 days later. I am very concerned that this will be around the time of main pilgrimage days. Do I need to take primulot again and from when should I Start. The side effects make me ill. Will it be worse on the second cycle

Dear XXXXXX,
As you mentioned that you have had spotting after taking primolut n on day 15, in that case thats the period by its scanty and only spotting as i mentioned its just the start of the secretory phase of the cycle.
Now the date of spotting becomes the date of your fresh menstrual period. so its fine.

Regarding your trip between 3rd to 7th october, you can opt for postponing your periods by taking a tab medroxyprogesterone acetate(MPA) 10mg twice a day from day 21 of this cycle daily till the day you dont want to menstruate. You can take it till 10th october. on stoppage you shall have your menses would have your menses within 7 days ideally and they may be heavy.
this heavy bleeding if lasts more than 3 days then can be controlled with antifibrinolytics easily. So you would be period free during your pilgrimage.
MPA can be prescribed by your gynae and its available on prescription.

Dear Dr XXXXXXX

I need to clarify. Today is the 6th day after last dose of primulot. The spotting ( don't think it is spotting is actually more clear slight discharge). If I understand your previous email, I would get period on day 15 which will be on the 22nd September. Will this be heavy?

The next bleeding after the 22nd September will it be 15 days later? Around the 7th of October? So will it be necessary to start meds again? Does the meds you prescribe have the same effect as primulot.

Thanks for the follow up again.

In my previous mail , when you mentioned that you didnt have spotting , which you now term as clear discharge, that was when i said you need to wait for 15 days atleast to resume your menses ( say 22 september). now this is well before your pilgrimage date of 3rd october.

if your periods come on 22 sept, then you should be safe during your pilgrimage and you may nor require MPA . instead you can then opt for ORAL CONTRACEPTIVE PILL from day 2 of menses till next 28 days. that should avoid your menses during the pilgrimage time and can cover it up.

but if your periods resume in next 2-3 days then you may opt for MPA from day 15 ( 1st october) daily 10mg twice a day for next 10 days to cover the pilgrimage period. on stoppage you can have your menses then . heaviness of menses again shall depend on the level of stress and hormonal changes in your body and cannot be definitely commented upon. But we usually appraise the women that a possibility exists and they need not stand alarmed.

MPA is a good drug and i have been using it safely on patients. but you need to get it prescribed by your physician.

I am sensitive to hormone medication. How does the mpa differ from primulot in terms of effectiveness and side effects

well the cycle is divided into 2 phases proliferative and secretory , and usually each cycle is 14-16 days depending on the length of the cycle,. So ideally you should get your next period after 15 days only.


As far as sensitivity to medication is concerned , if you can tolerate Primolut-N , then OC pills wont harm you at all.

Its a commonly used drug and my experience have been good. Primolut n has been seen in my practice to cause more side effects than MPA. so its preference . anyways you have to get it prescribed from your gynae , it would depend on his experience as well.
