Question: Followup question for Dr XXXXXXX Madhyashresta (Nephrologist)

Sir, with your guidance my father is doing well...now i have queries regarding some opprtunistic infections, cough and cold he may contact with.

please guide which medicine i should avoid and which can be given for below conditions

Viral Fever
Dry Cough
Wet Cough
Fever
Cold
Upset Stomach

I am not sure I understand the question. Is your father currently suffering from the following conditions or is the question for future possible issues?

It is hard to tell the medicines to avoid. In general most drugs can be given with slight dosage modifications. However drugs to avoid completely are painkillers or NSAIDs group that is diclofenac, aceclofenac, etc. I also suggest to avoid potassium sparing diuretics like spironolactone.

Hello

Thanks for the clarification. Here is what I suggest

1. Initiation of antibiotics should be early as CKD patients are immunosupressed. For viral fever I suggest Tab Paracetamol 500 three times a day. If the fever does not subside in two days I suggest starting appropriate antibiotics based on the symptoms and signs

2. For dry cough tab levocetrizine 10 mg twice daily is sufficient

3. For wet cough I recommend starting antibiotics like augementin or tab cefixime twice a day. I usually start an expectorant like syp ambrolite s 2 tsp twice a day

4. Upset stomach is a vague term. For gastritis I usually start pantoprazole 40 mg once a day. Please note sucrafil cannot be given in CKD

can my father have red wine/scotch?

his creatinine is 2.2

if yes then what is allowed limit?

Alcohol is not contraindicated in chronic kidney disease. He can have 2 drinks of his choice in a week and it will not alter his creatinine. There is no definite limit allowed as such but it should be in moderation
