Posted on Sat, 6 Sep 2014
Question: Doctor, I was diagnosed with Tuberculosis 2 days back. The ESR value is 30 and chest X-ray shows half portion cloudy in right part of chest. In a 3 month old X-ray nothing was visible. Thus disease seems to be less than 3 months old. My doctor prescribed Akurit 4 - 3 tablets after breakfast + Pyridoxine Hydrochloride 20 mg. Please suggest whether this medication is fine or need to get second opinion. I have no symptoms like cough, cold, fever, etc. But loosing weight and also I am diabetic. Age=34, weight=50kgs.

Please attach chest X ray pictures

Detailed Answer:
Hi XXXX,
Thanks for writing in to us.

I have read through your query in detail.
Tuberculosis is a chronic inflammatory disease and is very common in XXXXXXX The chief symptoms of TB are loss of weight with mild fever. Chest TB patients report cough exceeding 2 months duration which is not responding to usual antibiotic treatment. TB affects people having diabetes more than the general population.

At first I would like to have a look at your X rays if possible. You can take pictures of the X rays taken 3 months back and presently and attach them to your query. In case of difficulty you can mail them to YYYY@YYYY

After seeing the X rays I will get a better idea of why your doctor diagnosed TB in you and started medications. What I suspect is that you might be having a collection of fluid in your chest cavity which is medically termed pleural effusion. However I can confirm this only after seeing your X ray pictures.

The medications given to you are correct for treating TB but confirmation of TB is most important first before starting treatment. If you have a fluid collection then it is better to take out little fluid from the chest and send it to a lab for analysis. This will confirm TB in most patients.

Should there be further doubt then it is advisable to consult a TB specialist doctor (Pulmonologist) for confirmation of diagnosis before starting treatment.

Please attach X ray pictures so that we can continue discussion further.

Regards,
Dr.Vivek
