Question: Hello Dr. Kakkar
Do you remember about my case. I was suffering from redness and inflamation or swelling at the tip of the penis i.e Penis meatus. After going through the whole history or case, you identified that i am suffering from balantitis not urethritis as i am not sexually active
You suggested the following medicine for this problem
1. Single dose of Fluconazole
2. Clotrin-b (Clotrimazile and Beclomethasone) for one week
I have taken these medicine as per your instruction. When i applied clotrin-b for 3-4 days then redness and swelling goes away. But from last 2 days i am again facing the same problem. Swelling or inflammation is coming again. I did see an option to upload the images on Healthcare magic. So I am also sending you the images
at the following mail id YYYY@YYYY with the subject line as Attention:Dr. Kakkar.
Doctor I am getting married next month and i am facing this problem again and again. I don't want to pass this problem to my partner. Doctor i don't know whether it is a balantitis or urethritis. I want to get rid of this problem. So please suggest best medicine or antibiotic that can help in get rid of both (balantitis or urethritis) the problems.

Thank you for the Images. I have reviewed the Images. That does help in being more specific about the problem. I can see that there is redness, swelling and inflammation at the tip of urethra. The urethral opening looks eroded.

After going through the Images and considering that you have had this going on repeatedly I would like to consider a possibility of genital herpes and in view of that I would like to ask you about your past sexual history even though you already mentioned that you are not sexually active now. When was the last time you were sexually active? Did you have different sexual contact (s)?
Genital herpes can remain clinically silent for months or years and can manifest as recurrent inflammation/ erosions on genitals.

Genital erosions can also be part of causes like autoimmune pemphigus etc and need to be considered specially since your condition responded to topical steroids. Pemphigus usually presents with involvement of other sites like oral mucosa as well. Do you have a history of oral mucosal involvement with ulcers/erosions?

Doctor Please find below the answers of your questions
1. I have not done intercourse or sex till now.
2. yes I had the ulcer in the mouth. But it's occurence is not frequent. I got total 2 0r 3 ulcer in my mouth in last 3-4 yrs. Doctor i don't think these ulcers are creating any problem for me as their number and Occurence is too much low.

This problem started 4-5 months ago when i cleaned the penis with soap. After that i am facing this problem again and again.
But doctor one thing more I am suffering from Acne vulgaris from last 10-12 yrs. Is there any connection between Acne vulgaris and the problem that i am currently facing??

Hi.

Thank you. It appears to be a non-infective cause, likely an autoimmune or allergic. It is certainly not due to acne or treatment of acne. I suggest you to apply a non steroidal topical ointment i.e tacroz forte 0.1% ointment. It is non-steroidal. You may apply it twice daily for 2 weeks.

Hello doctor

Yesterday i did not take any medicine or apply any ointment. Today i have minor swelling on the penis meatus. I am drinking plenty of water today

1. Is it safe to use tacroz forte 0.1% ointment ?
2. Is there any side effect of this ointment?
3. Is there any antibiotic that can cure this problem completely?

Hi.

Yes, it is safe to use tacrolimus on genital mucosa.
There can be a smarting sensation on application in a few.
I don't think it is an infection and therefore I don't think that there is a role of an oral antibiotic.
