Suggest Incomplete Healing Of Wound After Fistulotomy

Question: Hi I had a procedure of fistulotomy (fistula ano) on 30th January 2015. Still the wound is not fully healed part of it still remains as such. Pus discharge happens even now also. I could see some pus in my stool as well. Since last week i feel my stomach is not clear, i feel constipated. I have to go washroom three to four times a day. Yesterday i saw few drops of fresh blood coming out of the operated area. Is there any problem what shoul i do. I asked my surgeon, he examined me and said everything seems to be fine.

Thanks for your query and an elucidate history.

To recapitulate: 30 / Male - Fistulotomy on 30 XXXXXXX 2015 - wound not healed - pus + - pus in stool also - few drops of fresh blood - 1 week feeling the stomach is not clear, has to go for washroom 3 to 4 rimes a day - on Orni-O - wants to know why and what to do -

Please give me some information for me to help you more:
Please post a clear picture / photograph (as I can not see in person so picture will do)
Since when were you suffering from the fistula?
Is there any one in the family who can do cleaning as I will teach you to?
Was this a low anal fistula or a high anal fistula?
Was the X-ray chest normal? if not done get a fresh one done.
Which city do you stay or work?

My thoughts and advise:
- Biopsy from the fistula tract to know the specific disease like Tuberculosis or Crohns as found in some patients (not all).
- Colonoscopy helps in patient with fistula which is not healing.
- Local care: Is there any one in the family who can actually helps to clean the wound and the area with Liquid povidone iodine and normal saline and diluted Hydrogen Peroxide.
- Maintenance of high hemoglobin, and very good Serum proteins particularly the Albumin is a must for good healing.
- Full normal diet as the other family take and you used to take before surgery.
- Therapeutic dose of Vitamin A and D and to be continued with Anti-oxidants (multivitamins) and probiotics.

All these factors combined together alone can make the fistula heal faster, better with best results.
