Brief Answer:
Please find diet and diabetes monitoring details below

Detailed Answer:
Hi XXXX,
Thanks for writing in to us.

I have read through your query in detail.
Please find my observations below.

1. Diet is an important concern for your father and you might provide him with a daily calorie intake requirement approximately 2100 to 2400 kcal. This can include vegetarian and non vegetarian food items as per your choice.

A sample daily diet chart is as follows

Early Morning

A cup of tea (sugarless) with 2 plain biscuits like XXXXXXX or Thin arrowroot or Rusk

Breakfast

2 parathas with curd (no sugar) or 2 plain rotis with egg/ paneer or 2 idlis with chutney/sambhar or 1 bowl of upma or oats

Mid morning

Soup or a whole fruit (apple or guava)

Lunch

Salad with rotis (2 to 3) or rice (1 bowl) with dal/ sambhar and a cup of vegetable. Occasionally you can add some light fires in lunch. Non veg can also be added (2 pieces) or an egg

Afternoon

Tea/ juice/ milk (sugarless) with 2 biscuits

Dinner

Same as in lunch with slight variation. Dinner should be lighter than lunch

2. Your father has a higher fasting and post prandial blood sugar level and with this the HbA1C is also to be measured once in three months. HbA1C is a blood test and gives average diabetes control for 120 days. Target HbA1C should be around 6.5 for him.

3. You have written that your father has diabetes for sometime but not mentioned any medicines. It is important that he takes medicine for control of high blood sugar level. On the other hand, if he takes medicine metformin then he needs to take Vitamin B 12 supplement because metformin reduces intestinal absorption of this vitamin and can cause deficiency state.

4. The potassium level is also to be regularly monitored. Foods such as melons, orange juice, and bananas are rich in potassium so please avoid its intake in high amounts if potassium is high.

5. The creatinine level is on higher side and with strict diet and medicine, it can be brought to normal limit.

6. He should also take medicine for his heart regularly.

7. Clinical examination of kidneys, eyes and nerves should be done every year.

Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek

Brief Answer:
Medicines are as required but diet need to be taken properly

Detailed Answer:
Hi XXXXX,
Thanks for writing back with an update.

The medicines given are good and he should take them regularly.
Please find my observations below

1. Can he take Spinach, Methi leaves vegetable
Spinach and methi leaves are good in diabetes and to be given in normal quantities.

2. Can he take green Beans vegetable

He can take green beans in normal quantities.

3. Can he take Besan ( Gram Flour) cury

Besan curry can also be given to him cooked in less oil.

4. Can he take 1 glass of Milk/ day & 1 Bowl of Curd/day

He can take milk and curds but please do not add extra sugar.

5. Can he take XXXXXXX ( Coarsely grounded Wheat)
XXXXXXX is good and recommended for breakfast.

6. In Fruits- Can he take Papaya, XXXXXXX juice, Orange, Tender Coconut water, & what specifically needs to be avoided.

He can take small servings of fruits about half to ¾ cup. Taking fruits is better than drinking fruit juice. Papaya is good for diabetes and protects the heart also. Mosumbi and orange fruit and pulp are better than juice and can be taken if potassium is normal. Tender coconut water can be taken occasionally about 1 cup.

Avoid fruit juices as it contains higher sugars and no fiber. Those with high potassium should take less of banana and oranges.

7.Pulses- a Bowl / day

Pulses can be taken but please cook in less oil.

8. How much water he can drink/ day which should not cause extra pressure on heart, as he had his CABG in 2004.

Also what should be specifically avoided to reduce cretinine, sugar, & balance Cholestrol.

Water intake should be in the range of around 2 liters per day. Proper diabetes control will help in reducing creatinine. For cholesterol I suggest you cook with olive oil at home.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek
