Posted on Wed, 12 Oct 2016
Question: Hello,

I am writing in to get rid of skin XXXXXXX (which includes face, neck, hands and legs most importantly very dark lips), which i am facing from long time. Very clear demarcation of color is evidenced. even my mom is facing same skin XXXXXXX as she is doing some gardening work. I tried all home remedies but never looked for approaching skin specialist. Please suggest me best medicine and how to use it (which should also help if i go for swimming). I am using faceguard sunscreen only suggested by one of the doctor.

Please do the needful.

Brief Answer:
Waterproof sunscreen and kojic acid on dark areas.Minimal sun.Check PCOS

Detailed Answer:
Hello XXXX, and welcome to HCM,
I am Dr Dinah and will be answering your query. Thank you for attaching your prescription details as well.
from your description, your skin seems more prone to tanning and pigment formation on sun exposure as you can see clearly demarcated.
You have also mentioned medical history of anemia and PCOS. Do keep in mind that anemia can cause your skin to look pale and have dark circles around the eyes, etc. PCOS can cause darkening around lips, face, neck, skin folds etc due to hormonal imbalances. I would advise to get these medical conditions treated well as they will greatly improve your skin and prevent these problems from getting worse.
If you have not checked recently, do check your hemoglobin and iron levels, and PCOS profile.
I have gone through your prescription in the attached file and see that you were prescribed Tenovate GN,which you do not need to use unless given to you by a dermatologist for skin allergy/rash with infection.
Continue the habit of regular sunscreen every 3-4 hours. Before swimming apply generous amount of water proof and sweat proof sunscreen available with Neutrogena, VLCC, Lotus etc. Reapply after swimming or sweating. For regular use you can continue your current sunscreen.
There are no effective short term skin lightening creams. Any cream must be applied for 1-2 months to see effects. You can try a kojic acid based cream for the face over dark areas only. Other options are skin lightening peels, LASERs etc.
As long as you are in the sun for long duration, even with sunscreen, your skin will XXXXXXX to some extent and slowly regain normal colour, so I would advise along with sunscreen, to wear cotton clothing that covers exposed areas for maximum prevention, if you must work in sunlight.
Try Dermadew Lite soap and After bath you can use detanning/lightening skin lotions to help.
Hope this answers your queries. Do feel free to contact us if you have any doubts.

Thank you so much Dr. Dinah for your suggestion.

I will follow the same.

You have informed to use sunscreen and skin lotion to further help from tanning. Can you please let me know how to make my skin normal now. I am sure about kojic acid. I am not sure if it is available in XXXXXXX Can you please suggest the brand name?

And I forgot to mention I was advised to use panderm plus cream to get rid of tanning. Before using I thought of taking suggestion.

Brief Answer:
Avoid Panderm +.

Detailed Answer:
Hello XXXX,
You are welcome.
Kojic acid is easily available as creams and gels everywhere. You can try Kojivit gel.

As for Panderm +, they are strong steroid creams used for other skin allergies and disorders. It causes side effects on skin on long term use, such as thinning, stretch marks, red spots and light patches. the whitenng is a side effect of the steroid. It is not to be used for skin lightening. It is wrongly advised by many.

Tanning over the face can be reduced by Detan facials and peels. This may not work as well over the thick skin of the body. Home remedies like lime juice, yoghurt, tomato etc can be tried.
Use skin lightening lotions after bath such as Nivea . It will take a while for the extra colour to fade away. And as advised, use sun protection.
Hope this clears your doubts.
