I read your question and I am sorry about the pain you are suffering.

You seem to already be under preventive medication with topiramate and take Norco (acetaminophen and hydrocodone) for acute attacks.

First of all let me stress that the drugs for the acute attack should be taken as early as possible in the course of its attack, the later the less likely it is to be effective.

Other treatment which could be tried are the triptans, perhaps you have already heard of them or tried them. They are the first line treatment and very effective. However they are contraindicated in patients with heart problems like ischemic heart disease and can affect blood pressure.

Other drugs which can be tried in the setting of an acute attack are antiemetics like Metoclopramide and Prochlorperazine. They can be taken by mouth and there also exist commercial forms where they are combined in the same pill with Acetaminophen of a nonsteroidal antiinflammatory drug for treatment of acute attack.
Dexamethasone or intranasal Lidocaine are used as well but have to be administered by a physician so can't be taken at home as you ask.
