Question: What antibiotics will protect someone in a hospital setting who is having respiratory issues and is in afib using amioderone in an attempt to convert?

I would recommend cephalosporines like cefuroxime, ceftriaxone or penicillins like ampicilline, amoxycilline, etc.
Macrolide antibiotics like clarithromycin, erythromycin can increase Amiodaron blood levels, and risk for toxixity in special conditions..
Kinolons like levofloxacine,moxifloxacine are contraindicated as their combination with Amiodaron can cause QTs prolongation and can be life threatening.
When facing with concomitant use of Amiodaron and other drugs like for example antibiotics it is of crucial importance to have a full and continuous monitoring of liver and renal function tests coupled with continuous ECG monitoring and frequent blood electrolytes checks.
Nevertheless, there exists a broad range of antibiotics with a good safety profile, so don't worry. What concerns would you have if Azithormycin was prescribed and administered two nights, knowing she was on Tikosyn and then went into AFIB on top of her respiratory issues?

Immediately after she went into AFIB, Tikosyn was immediately stopped and Amioderone started while the Azithormycin was continued. Any risk?

avoid azithromycin with Amiodaron

Hello! As I first explained to you Amiodarone used concomitantly with macrolides can highly increase the risk of QTs prolongation and life threatening arhythmia. So I would not recommend combination of Amiodarone and azytromycine which is a macrolide.
Tikosyn has the same serious interaction when combined with azithromycin. They both cause QTs prolongation.
Other alternatives should be tried.
