Neck Rash, Itchy, Bumpy, Have Dandruff. Is It Tinea Corpus, Rosacea Or Ringworm?

Question: Hello there,
Good afternoon,

I have this question related to my neck rash which started almost 6 weeks ago. Its started with small round rash then it turns to be bigger one with dark red outer and white center. I have this on 2 sides of neeck. I went to GP she said it could be tinea corpus so i applied cream for 2 weeks no luck. But i do see rash on left side of neck seems to dry since i dont see any scaliness there but it turns to be brown now may be its drying off but on the right hand side i have like 5 big rashes still red since 6 weeks. I am not sure what it is. GP said it cant be pity roscea since it should be like xmas tree and my rash itch. i have dandruff issue in hair not sure if it has to do with it. But this rashes are pretty big. And little bit bumpy when i move my finger i can feel there is a round rash.
I think its not rign worm either coz it shoudnt be taking 6 weeks to clear
Any advice on it?

I understood your problem however it would be good if you can answer few of my queries:
1) Is it only on the neck or do you have it on chest and back too?
2) Does it itch?
3) Doest it get aggravated on exposure to sunlight?
4) What was the first cream applied by you and what was the response(since it was diagnosed as tinea corporis)?
5) Did you take any oral preparation for the same and what was the response?
6) You said you have been using a anti fungal cream right now.I would like to the composition of it and how is the response following it?

I shall address your concern based on your current given information.
There seems to two possibilities in your case.One is a fungal infection which is pretty common in this season & over neck.
The other is Ploymorphic light eruption(PLE) which is nothing but allergic reaction to sunlight(again common in this season & over sides of neck).

If you can reply me with the answers and the pictures, I would be able to suggest you better.

As far as pityriasis rosea is concerned there is a very typical distribution in that over the back .It usually starts with a big mother patch over neck and then gets multiple small rashes over back in a "Christmas tree" pattern.This is usually associated with sore throat or fever.It has a viral etiology.

You can even consult a competent dermatologist for physical examination, evaluation and to get proper medicines prescribed by them.

Dandruff doesn't have a relation to what you have right now.You can use any good ketoconazole 2% shampoo twice a week for that.

Ans) Well on chest i had like small red dots not lot here and there but i think i had them before too. Its mostly on neck both sides of it. I have posted image showing it

Ans) It use to be when it started

Ans) Well it get worse after gym i mean i get more iching after that. But i normaly wear Shirt so that part normaly dont get expose to sun

Ans) I used Lamisil Cream. Before that i used Canstin antifungal when i first saw it. Honestly no response thats why i went to GP coz it got bigger.

5) Did you take any oral preparation for the same and what was the response?
Ans) No
6) You said you have been using a anti fungal cream right now.I would like to the composition of it and how is the response following it?
Ans) Yes Lamisil, Terbinafine hydrochloride 10mg/g

I tell you brief history behind rash.
One day when i went to work i saw few round rashes on my neck i stil surprised how i dit noticed before not sure it started over night or day before. It use to be tiny. But it got bigger over neck. After that i had few other small rash and they never got to be bigger. Only these ones.
We have cats and dogs at home but no one else in family has any issues of allergy.

More over i do have dry skin since we had pretty much like 4 months winter already and its started when winter started i never had this before in my life.

Thank you for the reply,

I have carefully gone through your answers & the image sent by you.The most possible causes would be first fungal infection (tinea corposris) & the second would be Ploymorphic light eruption(PLE).

In such situation I would advise you to take oral antifungal (like terbinafin or fluconazole ) for at-least 2 weeks and apply a combination of clotrimazole and betamethasone/mometasone cream (like HHZOLE) over the lesion for 2 weeks .

Oral anti fungal ,might not be an OTC drug I guess and thus would require a dermatologists prescription .Otherwise getting HHZOLE cream shouldn't be a problem and I just hope that it is available at your place.Kindly let me know the response of the same.
