Question: A 10 year old boy has a marked swollen area over his right hip which is very noticeable from the back ....from the .front his body is somewhat misaligned through his torso. There is no discomfort on palpating, no redness tenderness, or warmth. He is active and only occasionally notices any soreness most notably when cycling and skateboarding .....a couple of weeks ago he took a blow to theabdomen on his bike and apparently may have bruised his pancreas.

The boy has suffered an injury to the abdomen a couple of weeks back. I would like to know if the swelling on his right hip developed after this injury or if it was present prior to this.

The swelling shows no signs of inflammation (as you say) like redness, tenderness or warmth. It seems like a benign swelling, may be a lipoma or fibroma. A simple excision and histopathological examination will help confirm the cause.

Meanwhile, apply ice packs locally and observe if the swelling decreases in size.

Injury to pancreas may be accompanied by blunt trauma to other vital organs in the abdomen. It is better to do an ultrasound of the abdomen to rule out the same.

yes..... the swelling over the hip occurred approximately 1 -2 weeks after a spill from his bike which drove the handler bar into his abdomen. |There is still a red circular ring just above the umbilicus from that injury. Wil forward photos are soon as I have them. Thank you

Thank you for writing back and sending me the pictures. I have taken a good look at them.

The red circular ring above the umbilicus seems like a fresh lesion. If there is itching in the lesion, then it may be Tinea or ring worm. I am saying this because impact of the trauma cannot last for so long and look red, like I see in the picture. Even if it is a superficial hematoma, it should clear off in 3 to 4 days.

There is obvious swelling in his hip region. It may be a hematoma in the flank muscle or it may even be an impact injury to the XXXXXXX abdominal organ. In both cases, it is better to get an ultrasound of the abdomen done and get treated accordingly.

A superficial muscle hematoma will subside with local ice packs and anti-inflammatory drugs like Tylenol. An injury to the XXXXXXX organ may require surgical intervention.
