Question: for dr. Rovena Murati

Hope you will be fine doctor. as you know my case history. i just want to discus somethings. as per my stomach its now normal. or you can say after 3 weeks i have s soft stool but i have no such pain , or any other problem. i am having no other isue related to stomach. i still use one capsule of probiotic per day. and i think that they helped my a lot in improving these things. now doctor my question is still related to same things and frea. i have some questions , which may b i have asked before but please answer me.
problem is i have lost weight which i have discused with you before in the previous session. and you told me that its not very much significant . but problem is everyone who see me can tell me from my face that i am weak . 5 month before i was in same condition but at that time my weight was ok. now i have decrease weight . i can feel my shoulder bones. or may be its my fear also. i know i have decrease seat thing i have not reduced my intake of food. now my real fear and question is , does hiv cause such weight loss? or its late stage problem? or does i have diabeties? means you know my previous results? or its just a weight loss due to stomach problems or depression etc . kindly reply.

I am glad that you are doing better with the other symptoms you had.

I can understand you are concerned for your weight but we need to know if this is really happening.You need to measure your weight every month and see if you losing weight.

Weight loss is a symptom of the end stage of HIV (AIDS stage ).Diabetes can also cause weight loss.
I don't think the cause is HIV or diabetes because your previous tests of blood sugar were near normal or just slightly high and the risk of having contracted HIV is very low to zero.
Stress and anxiety can affect your weight.I would suggest to first try to eat well and healthy.If necessary increase the numbers of meals you eat and the amount of food you take.Try not to think all the time about your weight.
Then measure your weight every month.
If you are losing weight ,you need to test your blood sugar again (fasting blood sugar and HbA1c ).
You need to get the HIV test done so you can get rid of the fear and the stress this causes you.

i will try to do hba1c test, but last time when i visit doctor he again recomended random blood sugar test. hiv test i am still not mentally ready to do. but i will test in near future.
one more thing that i want to discus , before i was using centrum multivitamin. means before stomach problem and all these issues. can i start this medicine again?

I am sorry for the delay.

I would suggest to do a fasting blood sugar or random blood sugar.If the idea of having HIV is still a persistent worry for you,get tested as soon as you can so you can put your mind at rest.

You can take centrum multivitamine if you want,for few months.It would be better to take all the vitamins from food ,but you can take it.
The only problem can be the possible side effects of the multivitamins, which can be stomach upset ,diarrhea etc.So if you start to have such effects ,you can stop taking it.

can you please explain a litte bit that why a fasting or random blood sugar test is necessary for me? and for HIV i will definitely do becasue in next one or 2 years i have to marry , and in this situation its not possible for me to marry. i discus everytime such issue to you because all the symptoms you know that i have observed , and if i think wisely you are absolutely right that i was having a very low risk . but as per my stomach problems , mouth sore(which i was having before) hard pimples, and twitchings etc all these symptoms combine together to give me depression and anxiety. but from last some month i am very much out of this anxiety because of my work load in job. thats why i was having normal stomach. i observe that this cankers in month were also due to eating cashew . which i eat last week and i got again a small one. is it possible that i before i was having mouth cankers due to cashew?
and the last i told you 2 month before i was having arm pain in muscles and during that pain i was feeling twitchings and jerks, and due to this problem i was very much in tention and thinking that its a nurological problem.but its was finished in one week. but sometimes i feel a little pain in arm muscles in same area. but its very slight pain. what could be the possible reason? twitching now is not in a specific area. its very less but i may come any where in my body.

Hello.

I suggested to do fasting or random blood sugar just to check the level of sugar in your blood ,considering the slightly abnormal results you had earlier.
You just need to check the sugar level from time to time (once in 3-4 months) just to make sure everything is OK.
Since a high level of blood sugar (diabetes or prediabetes) can cause weight loss, it can be a good idea to check it form time to time.

Mouth sores can be caused from eating cashews or acidic foods.Stress makes them worse.
Considering all the progression of your symptoms ,the muscle twitching you were having are just benign muscle twitching ,which happen with no known cause.The pain in your arm ,seems to be just muscle pain due to a strain muscle .It is not of concern and not related to any condition.

thanks for reply. now please tell me hows that possible, i dont have diabeties but hight blood sugar can cause weight loss? and one more thing i am having now again one pimple on neck which is hard. is it possible that i got these pimples due to stress? but such hard and proper pimples can come with anxiety or stress?

Yes, having high blood sugar can cause weight loss.I don't think this is the cause in your case ,because your blood sugar is pretty much in normal range (the tests you have done).
I suggested the test just to know for sure the level of your blood sugar.

A pimple on the neck it is not indicative for a problem.It is just a pimple and it can be triggered by anxiety ,stress, food,environment,weather etc.

Stop worrying about this.It is not of concern.

I am glad that your blood sugar level is normal.

Random blood sugar and HbA1c are in normal range.This means you don't have Diabetes or Prediabetes.
The symptoms you were having of muscle twitching ,canker sores or stomach symptoms , are not related to Diabetes.As I have said these are common problems ,that can happen without any obvious cause.They are not indicative for HIV.
Your muscle twitching seem to be just benign muscle twitching,which can happen spontaneously and get worse from stress.
Stomach symptoms were probably related to acid relfux ,which is closely related to diet ,lifestyle and stress.

Since your blood sugar was in normal range, you can increase your food intake ,if you want to increase your weight.Always eat healthy foods.
Try to relax and manage your anxiety.
