Is Small Bruise Under Skin On Temple Post Injury A Serious Cause For Concern?

default
Posted on Sat, 7 Mar 2015
Question: My daughter fell and hit her temple. There is some bruising and inflammation to it. I validated her condition against this criteria from Mayo. She does not have any of those symptoms. It just happened within 15 minutes of this post. Should we seek the ER or wait until the morning?

Seek emergency medical attention if your child experiences:

Unconsciousness, confusion or disorientation immediately after a head injury or after some time has passed
Also seek emergency attention if your child exhibits the following signs or symptoms after a head injury, which could be signs of a concussion:

Persistent or worsening headache
Imbalance
Vomiting
Memory loss
Mood changes

Brief Answer:
If no symptoms of serious head injury then can wait till morning

Detailed Answer:
Hi XXXXXXXX,
Thanks for writing in to us.

I have read through your query in detail.
The picture shows your daughter might have a little bleed below the skin. It does not look serious but your daughter needs to be kept under watch for symptoms of head injury as per guidelines.

You have done the right thing by validating the head injury criteria as detailed by Mayo.
Head injuries need to be observed for the first 24 hours. The following are alarming symptoms (within first 24 hours) of a head injury that needs a doctors attention at the earliest.

1. Vomiting
2. Seizures
3. Persisting neurological deficits (numbness is a neurological deficit)
4. Difficulty in maintaining gait
5. Vision problems
6. Increasing severity of headache
7. Sudden behavioral changes.

If any of the above symptoms have occurred in the first 24 hours then you need to see a doctor in the ER immediately and get a CT scan brain done for your daughter if required after clinical examination.

If none of the symptoms listed above have happened then you can wait until tomorrow morning to reach the ER. Applying ice should help to bring down the swelling. However please continue keeping a watch on her and know if she has any of symptoms listed above.

Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Wishing your little one a quick recovery.

Hope your query is answered.
Do write back if you have any doubts.

Dr.Vivek
