Is Size Differentiation In Both Kidney A Sign Of Kidney Failure?

Question: i have 2 times ultrasound in which my right kidney is measured 10.0*4.4 and left kidney measures 8.6*4.1.
in second utra sound the difference is also same 2.4 cm.i mean my left kidney is small in compared to my right kidney.

ultrasound reports are below

-1 - small left kidney with prominent pelvicalcyceal system.Mild fullness of left pelvicalcyceal system (left kidney is 2 cm small than from right kidney )
2-cystitis with significant PVRU S/O chronic retention.

recent tests i have done-i have attached the blood and urine reports.
page 1 and page 7 says all my urine and kidney function resluts.i had done these tests 20 days back.do i need to to do any other tests?




I am very afraid...is it some signs of kidney failure?can u please suggest what may be the cause and how can i be treated.can i be treated only with medicines by consulting normal urologist.

I have read through your query in detail.
Being a radiologist I see many patients like you almost every day with questions that you have.

There is no need to get worried as your serum creatinine and blood urea are both normal. Urine tests are also normal.

The kidneys can differ in size up to 2 cm from each other. Usually the left kidney is larger than the right one but it can also be that the right is larger and this is acceptable . At 86 mm, I will not call the left kidney small if the right kidney is 100 mm. Only if a kidney is below 80 mm in length will I mention it as a small left kidney.

Coming to the prominence of pelvicayceal system, if this is seen in more than one scan without the presence of any infection in your kidneys or bladder then this can also be taken as acceptable. Sometimes even a normal kidney can have a prominent pelvicalyceal system and the radiologist must be careful before making a conclusion. A history of any kidney stones or urinary infection is important when reporting such findings.

I would like to know more about the post void residual urine (PVRU) volume and also the pre void volume when considering a cystitis (urinary bladder infection) and the thickness of urinary bladder in distended state. These are important parameters which need to be known before making a diagnosis of cystitis. The prostate size is also important but mostly in elderly patients.

Finally when giving an opinion on ultrasound findings, it is important to know your symptoms, especially urine related.

Please attach a copy of your ultrasound report so that I can go through it in detail and help you understand your situation in a better way.

The 1st ultrasound report report says that my left kidney is 1.4 cm smaller than right one.however i did one second ultra sound 1 month after the 1st and there it shows the left kidney is 2.4 cm less.does kidney size decreases or vary in every month or in years?am bit worried.the blood report which i did was 1.5 montha back..do i have to repeat again?

latest Ultra sound report details on tests are below.

right kidney measures 9.9 *4.5 cm and left kidney measures 7.4 *3.5.mild prominence of pelvicalcyceal system.bilateral kidneys are normal cortical echoes are normal.no calculus or mass lession is seen.ureturs are not visualized.


urunary bladder- this is well distended and normal is shape.no calculs or mass lession seen.wall is diffusely thickened (4.4_m.pre void measures 255 cc.post void measures 146 cc.

prostate- measures 21 gms approx.this is small in size normal outline and echo pattern.no focal mass seen.i never kad any kidney infeactions/urine infections or stone before.

please suggest if this is a kidney issue or any other issue?can i be cured by medicines if required..am worried

Thanks for writing back with an update.

The left kidney size is difficult to asses by most radiologists. This is due to presence of gas in the area and the size of left kidney measured at 74 mm can be due to this reason. I am saying this because a kidney will not reduce in size if reports are normal.

Coming to the urinary bladder in second scan, there are features of infection in the urinary bladder and because of it the urine is not able to come out completely. This should be causing pain and burning during urination and passing urine frequently.

The prominent pelvicalyceal system can also be due to infection.

With above findings I suggest you get urine routine and culture and sensitivity test done and then start taking medicines including antibiotics from your doctor preferably a urologist.

It is important to first give urine sample for culture and sensitivity before taking antibiotics otherwise the report will be normal and you will still have the infection and treatment will not be proper.

Also tell your doctor about any pain in left kidney area and fever.

Your kidney is otherwise normal on the day of scan. However any infection in the bladder can cause problems in the kidneys irreversibly if not treated for ten days. So please consult urologist immediately and get the diagnosis and treatment as required. If there is block of urine in the kidneys due to infection from bladder than it is more difficult to treat with more chance of complications.
