Is Pregnancy Safe To Continue With Dysplasia?

Question: my daughter has dysplasia/ cin3 and she has found out that she is 5 weeks pregnant is it safe for her continue with the pregancy and have the treatment after the pregnacy or is she putting herself at risk?

Thanks for writing to us with your health concern.
Generally, a cervical screening test is not performed in pregnancy because the hormonal changes greatly influence the result, and in a majority of cases, the severity of the lesion is downgraded when the woman is re - screened post delivery.
However, if this result was from a smear taken before pregnancy, or inadvertently taken during pregnancy, relax.
CIN 3 is a severe degree of cellular change, and in non pregnant scenario, should be promptly biopsied.
In pregnancy however, more conservative practice is implemented.
She should be followed up with regular colposcopies to see that the lesion does not progress.
If, God forbid, the colposcopy shows that the lesion is progressing into worse scenarios, then promptly biopsy would be done, irrespective of pregnancy, as a last resort.
Right now, she would be followed up with regular colposcopies and then screened again at 3 months post delivery to assess the cervical cellular picture.
Do not worry and stress out.

Hello

Thank you for such a quick response her last period was the 11th XXXXXXX and I believe she had a smear test on the 23rd XXXXXXX she was not aware that she was pregnant at this time.

She is so stressed (and me) she was booked in to see a consultant yesterday as a result of her smear test during the consultantion we were told that because she was pregnant no treatment would be carried out, she had a colposcopy where the consutant confirmed that she had cin3 and she did need treatment but this would now be carried out after the pregnacy, however if for some reason she had a miscarriage to contact them straight away so she could be treated.
She is booked in to see them again on new years eve but she has cried ever since receiving this letter because she is so worried that it will get worst while she is pregnant and not receiving treatment.
She was told not to worry and to go ahead with the pregnacy but of course she is extremly worried (and me) and just wanted a second opinion. Are there different levels at the Cin 3 stage or is it just Cin 3 and if the consultant had any doubts about it getting worst would she of advised her to have the treatment yesterday even though she was pregnant and god forbit if it did get worst how does this affect the baby if treatment had to be carried out? and do you know what % of pregnant woman have had to have treatment while pregnant because it has got worst and the outcome.
My daughters life would not be put at risk would it?

YEs, I second the decision that no treatment, except surveillance, would be carried out during pregnancy.
She has just had a colposcopy and she would regularly continue having them, to monitor the CIn.
If she miscarries, obviously the treatment is expedited, but otherwise, it can wait until delivery, and no, her life will not be put at risk.
Regular surveillance would be carried out, and at the slightest ominous change, the next step would be taken.
Crying and stressing out so early in pregnancy will take more of a toll on the baby, you need to be the pillar here and present her with scientific facts, and reassure her.
All doctors are concurring on this, and it is for her best.
CIN 3 is CIN 3, there are no different levels.
IF it was a frankly invasive tumour, she would have been treated irrespective of pregnancy.
It is because it can be watched that it is being watched, not to put her at needless risk.
IF it worsens, treatment might include procedures on the cervix which increase chances of miscarriage, preterm birth, Caesarean section etc depending on the stage in pregnancy at which the treatment is done.
Very few women are treated during pregnancy, because very few lesions worsen to the extent that immediate treatment is needed, less than 10 % of cases.
Please do not stress out, as scientifically speaking , her life is not at risk, and she will be under constant medical supervision.
Celebrate the miracle of pregnancy !

Last one she is booked in for 31st dec for her next check up which is 5 months away do you feel that she should have it checked another before Dec?
