Is It Safe To Have Back Surgery While Having Stents In Heart?

Question: Hi, I had three stents (two were xienc everolimus eluting coranary stent and one promus premier everolimus eluding platinum chromium coronary stent) put in my heart one year ago. I also have spinal stenosis and was looking to undergo back surgery. Do you think it is too soon to have the surgery, especially since one of the stents goes through the main artery. If I have to take the back pain for another year, I will. What is you professional opinion?

I understand your concern, and would like to explain that after a year from your drug eluting stents implantation, there is not any limitations (posed from cardiac issues) on planning an immediate surgery (if indications are clear).

After a year of double anti-thrombotic therapy (Aspirin plus Clopidogrel), coronary stents seem to be fully endothelized and the risk of intra-stent thrombosis is extremely low.

So, Clopidogrel may be safely interrupted, and the risk of bleeding from a required surgery is avoided.

The two key issues to be clarified before planning a surgery in these circumstances are:

(1) A clear indication for an immediate surgical intervention necessity.

(2) A current review of you actual cardiac performance; that is to rule out a possible refractory heart failure (which would be a clear contraindication to a near surgery).

You need to discuss with your attending doctor about these issues.
