Hello Dr

I am a 57 year old white male. My weight is about 193 and I am 5'8. I dont really look overwieght, however, I know I could get down to about 180lbs . Here is my question: I have just started Lasartan 50 mg per day for my blood pressure. Before the medicince, unless I ran everyday, I found my blood pressure starting to creep up again. Before the medicine I ususally ran about 150 over 95 than rarly higher than that. On the medicine I am running about 142 over 87 sometimes slighly lower. Hypertension runs in my family but do I need increase my medicine? I feel fine at this presure but is it doing me any harm? I had a recent normal stress test with the exception of a slightly faster raise in my heartrate than they would like to see but everything else is was normal. I have been on lasartan for about 4 weeks , should I give it more time before an increase dose? is any damage being done when my presure is 150 or 160 over 98 or 100?

Dear sir,

Thanks for the query.

I have read your history with diligence and noted the same.

1) You need to reduce your weight. Weight reduction helps in keeping hypertension under control. Persistently higher blood pressure does affect cardiovascular structures, however as your last stress test was normal, there seems to be no significant damage yet due to your earlier blood pressures.

2) Your latest blood pressure readings are still slightly high. We need to aim it at 120 by 80 which is the latest guidelines.

During your stress test you said that the heart rate was high - How much high?
If they were significant then another group of medicines called beta-blockers can be added to losartan to control both hypertension and high heart rate. This combination is the best way to reduce hypertension and protect your heart.

3) If I were to be your treating doctor, I would start tab metoprolol in addition to losartan. This combination is good for the heart. Discuss with your treating doctor.

I hope this answers all your doubts. Let me know if you have any other queries.

When i started the stress test they said the heart rate increased slighltly faster than they would like to see, however, after my test my heart rate recovered in normal time. I walk 2-3 miles a day should I start running a little bit to build stamina? My presure this morning 10-20-12 is 136 over 88

Dear sir
Your blood pressure is well under control. You can start by walking for about 50 minutes daily. If you do not have any issues, we can slowly build it up to jogging.
Please consult your local doctor for starting metoprolol .
