Question: Good morning, I spoke to you in the past about son who was going to go to XXXXXXX XXXXXXX We were reverted due to medical regency to Cleveland clinic right here is fl where we're from. My question is: yes it was positive for reaction to celexa in nov records. Then he was placed on lamictal also. We recently had them withdrawal lamictal. They did a 5 day study EEG negative again. But did state they took him off lamictal cold XXXXXXX and now after all negative results stated to start him on a new drug called vimpat. What is your opinion to wait and see if he has another episode or try this with precaution? He has renal tubular acidosis type 1 with severe reaction to lamictal and small starting case of XXXXXXX XXXXXXX syndrome. They wanted to do MRI but declined to do it now till after they get him on this medicine. Please send me some advice?

Brief Answer:
Vimpat (lacosamide) is safe.

Detailed Answer:
Hi,

Thank you for posting your query.

I am glad to note his negative test results.

Vimpat (lacosamide) is a relatively newer anti-epileptic drug (AED) and I find it a safe and effective AED. I have mostly used it as an add-on drug (along with another AED) in a difficult to control epilepsy and it has worked in most cases.

I feel it may be better to start the drug, as a recurrence of seizure may cause him physical harm.

Although the drug has a good safety profile, we should closely watch for any adverse effects especially with regards to renal tubular acidosis or skin reactions (though I have not seen or heard of them yet).

We can wait for MRI.

I hope it helps.

Best wishes,

We have decided since your comment was to be careful about the renal tubular acidosis were going to try going back to depakote for now. Thank you for your opinion. He didn't have a reaction but they changed the formula from powder to sprinkles that were rough on his mouth and gums. They are going to try the powder again. So pray it works and will be in touch in future with updates. Thank you again for your expertise.
