Brief Answer:
It is an open sore

Detailed Answer:

The chancre of Syphilis is an open sore be it in the anal or the genital region. The Chancre is usually painless unlike other STDs (Sexually Transmitted Diseases).

Pimple like structure with fluid in it is usually Herpes that is more often painful.
