Yes, clear blue fertilty monitor is quite good and effective tool to determine ovulaion time. It gives more than 99% accurate result as ovulation indicator tool.
Therefore, if it indicates that you are ovulating, you need to prepare for unprotected sex to maximize the chance of natural conception. You may undergo one home pregnancy test immediately after missed period to check pregnancy status.

Hi , I have been trying to conceive for the past 6 months. I am 37 years of age and overweight. My Doctor sent me for an ultrasound scan and is quite happy that I should conceive naturally but I am worried to death that something is wrong. I will type what the report says:

US Pelvis transabdominal & transvaginal

Poor quality image with limited resolution due to patient body habitus.

The uterus appears normal in size and echotexture with an endometrial thickness of 6.4mm LMP - day 27.

The right ovary has a volume of 10cc, the left ovary has a volume of 11cc. Both ovaries contain numerous small follicles. PCO cannot be excluded, suggest correlation with clinical biochemistry. No free fluid seen.

Do you see that I should be able to conceive naturally with the above report? What should my endometrial thickness be and my ovary volume?

Last year my cycle was roughly 27/28/29 days the past few months it is now 26-28 day cycle. Do you think I should conceive naturally?

My husband wont have a semen analysis but he does have a child to a previous relationship. He does drink and smoke but he is not alcoholic and has cut this down to see if this helps.

I am really worried as I am now 37. I have had unprotected sex since I was 25 but have never had regular sex, only had regular sex for approx 6 months. I have always had the same partner who is now my husband. Do you still think I have a good chance to conceive naturally. My own doctor says I have PCO but he says as I have regular periods and Clearblue fertility monitor plus day 21 progesterone shows ovulation. On the occasional time I have had no ovulation but always get regular periods.

Thanks for your help and please reply asap as I am going mad.

Thanks for follow up query with detailed history and test report. Followings are my comment regarding your queries:

1) Your ultrasound scan report is perfect with normal uterine echotexture, endometrial thickness (ET) and ovarian volume. ET corresponds to day 27 of your menstrual cycle.

2) Small follicles on both ovaries may be functional type. It does not impact on your pregnancy as you have gotten regular period. Also, D-21 progesterone hormone assay confirms normal ovulation.

3) Your period cycle is also normal and you can successfully conceive naturally at your age if following test reports are normal.

a) Tubal patency test b) Husband semen analysis c) Blood sugar and thyroid hormone profile d) Test for cervical factors- Post Coital Test (PCT) & Anti Sperm Antibody (ASA) etc.

You need to regular unprotected sex close to ovulation or fertile days (use clear blue fertility monitor tool) to maximize the chance of pregnancy.

4) Drink and smoke may cause abnormal spermatogenesis & semen analysis may not be always normal even he had child in previous relation. He needs to quit those habits.

In addition, you have to try 6-12 months continuously for natural conception. You would take folic acid and Vitamin-E supplement during preconception stage and thereafter. Take low carbohydrate diet, do regular physical exercise to control body weight (have great role in conception) and avoid mental stress by regular meditation or yoga.

If you fails to conceive within 12 months, you must consult with infertility specialist for next line management.

My husband wont give up smoking a drinking but he is cutting it down by half. He has gone on roll ups instead of cigarettes and this cuts him down dramatically. He is now drinking within the alcohol guidelines. Will this help with his sperm count? Can you please let me know what the guidelines are for a man please, I dont drink or smoke so that is fine but I am overweight and am trying very much to lose weight.

Thanks for your response.

Spermatogenesis is commonly affected by smoking, alcoholism, hot working place, gross obesity, systemic diseases etc. He must cut down alcohol intake and smoking to improve sperm quality. Do regular physical exercise, take balanced diet with vitamin-E & Co-Q enzyme supplements. Here, one semen analysis test is mandatory to check overall status.

Normal Semen analysis guideline as per WHO 2010:

1) Semen volume: > 1.5 ml and PH: > 7.2
2) Sperm count: > 15 million/ ml and Total: > 39 million/ ejaculate.
3) Normal sperm morphology: > 4% (strict criteria)
4) Sperm motility: > 32% (progressive) and Total: > 40%
5) Liquefaction time: within 20 min after ejaculation.
6) Fructose: > 13 mol and Zinc: > 2.4 mol per ejaculate.
6) Live sperm: > 58% and Pus or white blood cell: 0-5/ HPF.

Lastly, you must do regular exercise to reduce body weight by means of yoga/ treadmill/ running etc. It has great role on natural conception.
