Suggest Treatment For Pain In Arm, Dizziness And Stomach Discomfort

Question: I was at the Dr. on 3/21/16 and bp was 129/74 (which is the range where is always tends to be). I went into the grocery store yesterday after work, grabbed a basket and noticed a quick sharp pain in my right arm, was feeling sick to my stomach and dizzy. I went over to the bp machine and it was 188/111. I decided to go get the items I needed and checked it again 20 minutes later. Dizziness was gone, pain in right arm was gone however still feeling the nausea. 2nd bp was 175/118. I am a 45 yr old female with no previous heart issues. Today, I have just been extremely tired, shooting pain in upper right side chest that shoots through to my back and then is gone in a few minutes. I have also had this feeling where I can feel my heart racing at times. I was going to follow up with my family dr. on Monday but just wondering if this sounds like a emergency or not???

Hello ,
I am Dr.Rajesh Teli,MD.Thanks for using HCM.
I have gone through your query.
As I understand that your previous regular BP check up revealed that your BP remained within normal range.
while you were at grocery store and having grabbed basket you had sharp pain.Many a times BP shoots up due to severe pain .(Any severe pain anywhere in body can raise BP).so I feel you should focus on detecting cause of pain along with BP monitoring.I feel your pain might be due to cervical spondylosis leading to radicular pain,Nausea and giddiness.However this is my presumption you need to undergo clinical examination and investigations for this.
I would like to know following information
1.Have you family history of Hypertension,Diabetes or heart disease?
2.Do You smoke or consume alcohol?
3.What about your menstrual History ?were all pregnancies/pregnancy Normal?
4.Did you have any major illness in past ?
5.Have you history of aciddity or stomach upset?
Having known this I can guide you further
Meanwhile I suggest you to take rest till monday ,don't lift weight or exert too much avoid spicy food and reduce slat intake.
I suggest you following investigations(depending on clinical examination you may not need all)
1.complete Blood count along with Blood sugar.
2.X ray chest PA,Xray Cervical spine and Ultra sound upper abdomen.
3.Resting ECG

Paternal grandfather had diabetes & passed from massive heart attack at age 62. Father has high blood pressure and had a stroke at age 40.

2.Do You smoke or consume alcohol?
I have never smoked. I do consume beer socially (2-6 times monthly; 3-4 beer at a time)

3.What about your menstrual History ?were all pregnancies/pregnancy Normal?
All 3 pregnancies delivered via C-section due to failure to progress-no dilation, head did not descend, water would not break. Babies were 8.3 lbs., 7.1 lbs, & 8.8 lbs. Everything else was normal. Hysterectomy 1 year after last baby due to excessive monthly bleeding, pain & fibroids.

4.Did you have any major illness in past ?
Mononucleosis & Pneumonia.

5.Have you history of aciddity or stomach upset?
Always had either diarrhea and/or constitpation up until 1 year ago when I started taking a probiotic daily. Prior to that, I had my gallbladder removed.

You have significant family history.So you must anticipate hypertension.please monitor your blood pressure and you may need antihypertensive medication.you BP should remain below 140/90.
I would further like to know following details
1.what is your weight ?Are you over weight or obese?
2.Have you stressful social life?
3.Were your ovaries removed or preserved during hysterectomy?
looking at your history now I would suggest you to first to undergo cardiac check up
1.get your complete lipid profile,
2.If resting ECG is normal get your exercise stress test done.
3.Ofcourse get previously suggested tests done.

I weigh 193 lbs. and I am 5'5"

2.Have you stressful social life?
Socially I bowl on a mixed couples bowling league, go to baseball games & hockey games, and/or generally just out to dinner with friends and family. Socially is actually relaxing and fun for me when I get to do it.

3.Were your ovaries removed or preserved during hysterectomy?
My ovaries were preserved during the hysterectomy.

You certainly need weight reduction,however at this stage first and formost important need is to establish cause of your pain and monitoring of blood pressure.
As your ovaries are preserved it means that you may not have entered menopause.Women are protected against heart disease till menopause.Post menopausal risk is equal in both genders.
Kindly get your cardiac work up and do inform me so i can guide you further
