1. During day time do you have any chest symptoms like chest pain, cough, fever, breathlessness, palpitations? Also since how long have you noticed these symptoms?

2. Due you have swelling in your legs, tummy?

3. Do you have associated cold, sneezing?

4. Do you smoke?

5. Did you ever have whistling sound during breathing?

6. Have you ever been told by anybody that you snore? Please mention your height and weight?

5. Previously did you ever have any chest complaints?

6. Do you have any other diagnosed disease in the past?

7. Have you ever had your chest xray, blood investigations done?

8. Do you have any type of allergy?

9. Do you have any significant family history?

10. Did you ever visit a doctor for this? If yes what was the result?
