How To Rule Out Folliculitis?

default
Posted on Mon, 7 Dec 2015
Question: Hello
46 year old male caught off guard 4 days ago with an over night growth that is in my perineum area and has grown ever since
Very swollen but dose not hurt other than the strange discomfort of it and is very tight from the scrotum to just outside the lower butt check on the right side.
I was wondering if this could be caused due to an n grown hair from the elastic on my underware as it seams to be following that area
Or is this something more ominous??

Hi
Welcome to Healthcare-Magic

Dear Sir

Going by description it could be folliculitis which is infection of hair follicle, exact diagnosis will however depend on good clinical examination. \
It would be very helpful if you could provide me with clinical photograph for further evaluation. You can mail me at YYYY@YYYY with subject line being "Attention Dr Shobha Deepak". Or upload attachments to upload report available on the website.

Thank for responding so quickly last night.
&
Please forgive the unfortunate authentic but graffiti nature of these photos.

The inflammation is an my right side. ONLY

I had some development last night about 3am with an area rupturing on its own.

The swelling of this area or vein has subsided and no longer feels as hot or angry.

For 3days it was very pronounced and the skin very tight.

The tightest area was/is within the scrotum with what feels like the (ISCHIOCAVERNOSUS) that's really the weirdest feeling down there because it's super inflamed feeling.

Other than discomfort it doesn't hurt the odd occasion I felt like scratching a lot.

As for meds I'm using

Tylenol
Hot compresses/baths
Today added
Polypsorin
&
Tea Tree oil

Brief Answer:
It is fungal dermatitis

Detailed Answer:
Hi
Dear Sir

Thanks for providing the clinical photograph. From the photograph it appears to be fungal dermatitis which is secondarily infected with bacteria.

I would suggest the following

1. Hot Seitz bath: Immerse the area in tub of lukewarm water for 10-15 minutes each session twice a day

2. Keep the area clean and dry

3. :Local Application of combination os steroid and anti fungal ointment will be helpful. I would suggest Clotrimazole (anti fungal) and Beclomethasone ( Steroid) thrice a day

4. Local antibiotic ointment for ruptured boil

5. Wear only pure cotton underwear, preferably avoid wearing any undergarments while going to sleep

6. Chlorpheniramine (Anti-histamine) will be helpful to relieve itching

7. Stop application of tea green oil as it will worsen the dermatitis

Do get back to me if you need any further assistance, will be glad to assist you.

Take Care

Regards
Shobha

Hi Dr XXXXXXX
I've already purchased what you've recommended and have begun the applications.

Need to ask questions...
I ask forgiveness because it's a little confusing.

Is the fungus on my skin the cause of the inflamed nerve/vein/tendon under the skin part of the muscle structure??
The area from underneath the scrotum along the pernieum through the lower right buttock.
I ask because that's the part that's super weird/uncomfortable I can feel it especially when I sit down which I can't do at the moment for very long before it becomes painful.

Or is this "thing" I'm feeling going to subside through the application of topical cream.!?

This is making me nervous can this fungus spread?
IE: touching the affected area and then touching another part of my body

Brief Answer:
It will not spread to other body parts

Detailed Answer:
Dear Sir

Fungal infection in groin area is superficial fungal infection . it is usually cause by excessive moisture and sweating in the area. It is not related to any tendon or nerve. There may be occasional burning sensation with first few applications of the cream which will usually subs. It is unlikely to spread to other body parts.

Rest be assured there is no cause for concern. Religiously following the previous advise will be very helpful.

Hi,
About 3 hours ago I've had what I would describe as a slow leaking stream of mucus/pus coming out of the area that popped Friday.

I'm patting it clean with warm water and small amount of salt every 10 min and placing polysporin on in in fear of infection.

I'm not sure this is just a fungal thing any longer because as the leaking progresses the taught like vein I've been trying to explain is becoming less taught.
As much as anything Ive tried explaining it almost feels like a hard banana like shape under the surface of the skin.
(I know that sounds crazy I wish it didn't but there you have it )

I think it's an Abscess.
Its the same everything I sent in the photos - no blood like the first time only clear to pinkish gelatonous material steadily oozing out and it does not have a strong scent either.

Maybe what I was describing was the abscess under the skin -

I apologize for my nonsense but I have never not have a Family Practitioner and did not want to clog up emergency needlessly.

I just want to make the best decisions for this stupid condition I'm in and not bother anyone any more.

Brief Answer:
There is secondary bacterial infection

Detailed Answer:
Dear Sir

As i have already mentioned its fungal dermatitis which has secondarily got infected leading on to localised collection of pus. I would suggest you the following additional measures

1. Continue Hot Seitz bath

2. Oral antibiotic like Augmenting ( Co-Amoxiclav ) will take care of the bacterial infection

3. Anti-inflammatory like Ibuprofen will be helpful

Do send me clinical photograph for reassessment

Take care
