How To Retain Premolar Teeth Roots?

default
Posted on Mon, 23 Jun 2014
Question: I have a 90% lost tooth ( First Premolar ).. I had done RC for that teeth and didnt put a cap.I assume it to be the reason for losing the major chunk

I prefer a RPD for the missing tooth without removing the left over tooth parts. Is it possible?

Detailed Answer:
Hello and welcome.
Thanks for sharing your concern.

your history suggests that you want to retain your premolar roots. Therefore before I explain you regarding its treatment aspects I would like to know few details. Please answer them to suggest better in this regard.

1.How is your periodontal(gum)health,I mean do you have any bleeding from gums?
2.Is your first molar tooth, next to first premolar is absolutely fine with no decay or filling?
3.Did you take x rays for the first premolar to check if there is any pathology developed under your fractured first premolar.

4.And lastly, did you feel pain or tenderness in your premolar or any other teeth in the same arch?If so please mention.

Awaiting response.
Thanks.
Take care.

Thanks for the reply!

For the queries, my answer as below:
1. Healthy. No bleeding/no pain ..nothign at all
2. Yes perfectly fine
3. No.
when i did my RCT, the tooth was only 60% intact...After a few months after the RCT when i bit on some food the remaining piece came out ( the tooth had turned grey- i assume all the roots were cut hence it turned grey) Its like the tree is cut,but the bottom part is still remaining
4. No .Not at all.

Brief Answer:
Advised post and core

Detailed Answer:
Hello and thanks for reply.

Based upon the history posted here now it is clear that you have healthy gums and teeth adjacent to fractured premolar.

Therefore there is a treatment called post and core which can be performed on this fractured tooth. This will save on the remaining tooth portion. You will have to meet an Endodontist (specialist dealing with root canal cases). He/she will know about the procedure I am talking about. The doctor might perform x ray evaluation and clinical examination before taking you up for post and core treatment.

RPD also called as removal partial denture, is done to maintain the gap between two or more missing teeth. This treatment is done only after complete removal of teeth.

Hope it helps.
Let me know for further doubts.
Thanks.
Take care.
