Hi,
I am looking forward to talk with doctor to discuss about premature ejaculation, Erectile Dysfunction and also seek advise further related to sex health. Along with that I need advise where, I can get the sperm count analysis done.
I am getting married next month and also want to have complete physical examination done as preventive option.
AS I am getting married next month, we (me and my would-be wife) has decided to plan for baby soon.

During all sex conversation between me and my wife, we both really look forward to longer sexual intercourse act.

I am based in Delhi, India..

I appreciate your efforts for medical consultation in so much distress.

Sexual dysfunctions can be a result of a psychological or physical problem.

Psychological causes: Most common cause in young patients. These include performance anxiety, emotional weakness, inferiority complex, stress, marital or relationship problems, low mood, feelings of guilt, and the effects of a past sexual and emotional trauma. Men with psychological problem usually have normal nocturnal erections and erections upon awakening, whereas men with physical problem often do not.

Physical causes: Include neurological disorders, alcoholism, drug abuse, hormonal imbalances, diabetes, hypertension, blood vessel disease and chronic diseases.

I need following information to help you better:
-Do you have morning erection sometime?
-How you come to know about these problems? Are you sexually active? If yes, then what is your ejaculation time?
-Do you have anxiety attack during sex or during masturbation or some other time also.
-How is your sleep and appetite?
-What is your primary sexual problem: Erectile dysfunction or premature ejaculation
-Do you have problem alcohol abuse, hormonal imbalances, diabetes, hypertension, blood vessel disease or other chronic diseases?
-If yes then, are you on any treatment for that

You can get your semen analysis from nearby lab like Lal Path Lab after 5 days abstinence from sex or masturbation.

Below are the answers to your question:

Do you have morning erection sometime?
Yes, I have morning erection sometime, but it doesn't stay longer, it just goes off the movement I pass the urine and morning erection is not everyday.

How you come to know about these problems?
I read about it and during masturbation, I end up discharge fast, more likely in lss than a minute. I sometime masturbation daily with an average of 15 times a month.

Are you sexually active? If yes, then what is your ejaculation time?
No, I am not sexually active, I had sex after 7 years of GAP because during the 7 Years .. I was fighting divorce battle and during last 7 years, it was just masturbation and nothing else.. I didn't had good physical relation with my ex-wife or you consider almost nil physical relation till we stayed together because she got married to me under family pressure whereas she was seeing someone else and I caught her red handed in bed with her longtime boyfriend from her office with whom she wanted to get married.

Till the time, her relation was not know to me, upon asking for sex etc, she used to always say had no desire for SEX and she need more time.

I am presently seeing another girl for getting re-married, she is widow girl, almost 8 years younger to me, she lost her husband in car accident on returning from her parents place to in-laws place. she got saved and recovered after 9 months of treatment.

We talk everything and anything about sex, we both has high desire to get in bed for sex. During her period this month, we had Oral sex and she endup masturbation me and I ended up very early.

Later, once her periods were over.. we had intercourse, this was for me after 7 years and I couldn't end up longer and I was done is almost 2 minutes. This brings lot of embrassement to me and now I am been asked by her is this normal or what ?

Also, during intercourse, I was going breathless for sometime, dont know why.. to add here, I don't do any exercise because of long working hours, odd timing, work load etc.

Yes, I feel nervous, with a thought of girl in today's world thought about sex, expectation when having sex, duration, act & action in bed etc.

During my 7 years, I have seen "N" number of couple seeking divorce because of sex as original factor in background. On cover.. they apply different story.

I dont get anxiety other time. Most of time I loose my patience.

I dont each non-veg, I dont drink alchoal, No smoke or gutka.
I cosume over 5 cups of TEA everyday. I travel a lot, so I need to adjust my sleep accordingly.

-What is your primary sexual problem: Erectile dysfunction or premature ejaculation
To my understanding and with current experience, its both because I loose erection quickly as well as ejaculated fast.

To understanding given to me by my would-be wife she expect long lasting relation in bed and we will intercourse once again.. I return from travel by end of next week.

Thanks for follow up.

From further information it appear that you do not have major problem. Normal ejaculation time is more than 3 min (even less if both partners are satisfied) and it take few hours for re erection of penis after ejaculation.

Following steps will be helpful for treatment:

- Try distracting yourself when you feel you are about to ejaculate. Take a XXXXXXX breath and stay relaxed during intercourse.

- Do not worry about performance and talk to your partner openly about any apprehensions if you have. Conversation with partner during sex also delay ejaculation.

- Stop-start or squeeze-pause technique- In stop start technique, near ejaculation withdraw your penis from your partner and relax and think about something else. In squeeze- pause technique, squeeze the end or the tip of the penis for 10 to 20 seconds when you are close to ejaculation and then resume stimulation after withholding for half a minute.

- If the symptoms persist then condoms with desensitizing creams (marketed as LONG LAST) and SRRI (selective serotonin reuptake inhibitors) like dapoxetine will be helpful, which you can get after prescription from physician.

-Ejaculation time also increases with experience in sex.

For semen, test name is: Semen analysis. Prescription is not must for this test.

Hello Again!

During intercourse, I was breathless for sometime, dont know why.. ? I am really concerned about same and don't want me see myself in that state. It took me almost 30 minutes to recovery to normal state, is this point to worry ?
It was almost like I don't have any stamina, what should be done or what practices to adopt better stamina so that I can give complete satisfaction to my wife.

I am really concerned about same and this concern is giving me breathless night..

How can I increase my stamina to perform much better intercourse to my would be wife.. post I return next week because we already had planned all up for sex upon return.

During her period this month, we had Oral sex and she endup masturbation me and I ended up very early in less than minute, is this normal even when women performing masturbation on me ?

Hello again,
Thanks for follow up.

For premature ejaculation follow above advice.

Breathlessness during sex indicate presence of anxiety. In that case you should seek in person assessment by psychiatrist as anxiety is also one of the reason for premature ejaculation.

You may also need medicine help, which can be given on prescription only due to ethical reasons.

Q. How do I explain her about my concern ?
Answer: Just tell her about difficulties, as you are telling to us.
