How To Control Diabetes And Depression?

Question: Hi. I have a friend with out of control diabetes (260-300 daily) and A1C of 14, 220 pounds and 12 heart stents. He is only 53. He will not eat a balanced diet and does not exercise due to depression over it all. What is his prognosis and life expectancy? He is stubborn and will not listen to advice.

Brief Answer: Prognosis is poor! Detailed Answer: Hi and thank you so much for this query. I am so sorry to hear about what your friend is going through. I understand chronic illness can be a cause of depression that further worsens the disease state. However, we need to be able to work even harder to make sure that we don't get there if we are serious about taking care of the disease. His prognosis depends on how well he would get involve in controlling his diabetes and other disease conditions.Should he not take any advice, then he may die even in weeks from a possible acute complication like diabetic coma or heart attack. However, with minimal efforts, he can live for years but in a miserable state because the disease would be a great burden to him because of complications but, none would be sever enough to kill him instantly. I will suggest that you try and get him more motivated and involved in his care. I believe he needs to be consulted by a psychiatrist for management of his depression and find ways to get him back to taking control of his health. In all, his prognosis is poor should he not change from his bad ways. It can lead to death within days or weeks from acute complications if he is totally negligent or may take years if he is stubborn leading to longterm complications and a very poor quality of life. Also, consider getting a psychiatrist consult for him. Does he still smokes? I hope this helps. I wish him well. Thank you. He has also been having constant deep chest pain since last three stents. He goes from lying around all week, depressed, eating one bad meal a day ( refuses to eat three and snacks) to overdoing it on weekends as he has 13 year old girl and boy twins who are very active on weekends. He and his wife do not get along at all so there is constant fighting to cause stress plus her they both lost jobs and the new one she has now pays way too low to pay all bills. His job is SBA loss verifier and there has been no damaging weather since XXXXXXX so he is not working. It is a train derailment so on top of everything they have money woes and stress. All he has to do is eat three meals a day to at least help a bit with sugars but he won't. There is no heart disease with his parents so am wondering if smoking pot for 20 years hurt his heart. He did give up smoking regular vigarettes 10 years ago. Also is there anyway you could summarize what you told me in a way I could pass it along to him. He thinks he knows everything and takes risks, like driving himself home on highway after stent placements....not wearing seat belt etc. Anyway is there anyway you could write something here that might scare him straight, out of his depeession and up and at it. Diabetes can be controlled simply and with good meals. I deal with cancer patients who don't have the second chance he could have and I am losing patience a bit. But he is so tired now all the time, not just from depression but things are seriously going awry as well. Also is there any chance shoulder and neck pain can be referred pain from heart or indicative of things going on there? Thank you

It can be difficult for the family to go through such hard times when there is little to spend and illness is also a big concern. I will like you to make your friend understand that he needs to take good care of himself. he may be negligent of is ill health and this would only open the doors for further complications that would make his life miserable. what many people fail to realize is that not taking care of yourself doesn't always result to death but leads to severe complications that worsen your already compromised quality pf life and that is something no one should wish for. Chest pain, shoulder and neck pain could all be referred pain from the heart. It needs thourough question and examinations in order to tell if that is the case. With his stents history, having an ischemic heart disease is a more likely than not. Should it persist, please let him talk to his cardiologist. I hope this helps. talk to him and stress not on death but on threatening complications that would not kill him but let him suffer so much like heart failure, liver and kidney failure, diabetic neuropathy, etc. This is what can make him think of reconsidering his efforts to stay healthy. Thank you. I have already talked to him about that. I volunteer in ER and have tried to tell him of pts with lost limbs and horribly infected feet and on dialysis every other day but it is a smoker who thinks someone else is going to get lung cancer, even tho he knows the neuropathy in his feet is symptomatic of damage caused by high sugars. Thank you for your time and help. Is there anyway I can get you if I post updates rather than Explain all this to someone new?
