Question: To be answered by Dr. XXXXXXX Hiremath” Hello Doctor, In continuation to our conversation today on the following thread https://www.healthcaremagic.com/AskDoctorInboxServlet?page=viewQuery&queryId=103027, i am writing to you with another question In your last response to me one hour back you told that "The highest risk is during sexual intercourse when the genital mucous membrane is exposed to semen or vaginal fluid which is very rich in HIV particles and HIV enters the genital mucus membrane through small abrasions that are caused during intercourse." So as i said in my case, when the hooker gave a blow job to me using a condom, do you think it can cause abarasions on my penis and the tip of penis, which later could have been exposed to the semen drops in the wash basin? Do you think this chance can exist? or this is a remote possibility? Also even though we did not have sex and only blow job with a condom on it, she used the same wash basin before me and may be she washed her hands and the vaginal area there at the same wash bin, after a few seconds i went in where i mentioned to you about my exposure to the tip of the penis... Could you please clarify this last thing before i can close my computer and head for a good night sleep, would appreciate if you could write to me soon

Brief Answer: HIV exposure Detailed Answer: Hi, Welcome back. There are too many assumptions happening here. Taking into account, the non viable HIV particles present on the wash basin(Viral particles do not survive on glass and ceramic surfaces for long), there is less likely chances of infection occurring through direct contact with mucous membrane of genitalia. Please do not worry unnecessarily. Instead I would suggest being cautious in the future and avoid high risk behavior. If you are still worried about exposure, I would suggest an ELISA for HIV 1 and 2 antibodies test three weeks from now. Thanks Doctor, I understand your advise and would not repeat this in the future. It was my mind playing tricks on me and i wanted to ensure all possibilities in the before leaving it back forever. To summarize our discussion, i understood that 1. HIV can only spread through unprotected ANAL/VAGINAL sex and through internal transmission of the virus internally in the body. 2. The virus will be dead once exposed to air and would not survive on glass and ceramic surfaces. So if you are exposed to the mucus membrane for a brief period of time lets say for 10 seconds, the chances are still very less 3. Apart from unprotected sex, blood transfusion, needle sharing, mother to womb, the chances of getting HIV is zero to nil using wash basins, public toilets etc even if your skin or mucus membrane is exposed to the virus. 4. I will assume the above and put my worries to sleep, i am satisfied with your very clear explanation and would skip the HIV test, as taking the test would cause unnecessary worry and still more new thoughts. For the HIV test part to confirm the negative status, i am preferring to skip it, as you ONLY recommended it to reduce my anxiety/worry which anyway is gone now after your previous mail.... Well thanks doctor i understand, However the reason i asked for is The process can be quite long as first i need to talk to my family doctor who in turn has to make an appointment with the STD clinic in Stockholm, Sweden where i live, our family doc knows me and my wife very well(a family friend) and i prefer to skip it to avoid embarrassment. Also i now i understand how it spreads and since i have had a low risk exposure, I would prefer to skip it, Would you still insist?
