Brief Answer:
Routinely given for hypertension,it must be taken with caution in diabetics

Detailed Answer:
Hi XXXX,
Thanks for writing in to us.

I have read through your query in detail.
OLMEZEST H 20 tab contains olmesartan 20 mg hydrochlorothiazide 12.5 mg.

Olmesartan is a medicine that is angiotensin II receptor antagonists. It keeps arteries from narrowing, and lowers blood pressure and improves blood flow.

Hydrochlorothiazide is a thiazide diuretic that helps prevent the body from absorbing too much salt.

Hydrochlorothiazide and olmesartan is a combination medicine is used to treat high blood pressure (hypertension).

Studies in diabetics has shown this combination of medication to be beneficial in controlling high blood pressure and reducing urinary excretion of proteins.

There is a word of caution for diabetics taking this medicine combination because rarely it might cause high levels of potassium especially in patients taking Aliskiren. If serum potassium level is in normal range then there is no reason to worry. Also few people might experience low blood pressure after taking this medicine and any dizziness or giddiness must be reported to your doctor. Blood glucose level is to be closely monitored when taking this medicine.

Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek

Brief Answer:
Your diabetes under moderate control, please find medication details below

Detailed Answer:
Hi XXXX,
Thanks for writing back with an update.

There was a large scale research on whether to give ACE inhibitors to patients with diabetes recently.

It was concluded that "ACE inhibitors should be given to all type 1 diabetic patients with microalbuminuria or nephropathy, and to type 2 patients with hypertension, microalbuminuria, or existing cardiovascular disease. All diabetic patients with hypertension and left ventricular hypertrophy should have an ARB such as losartan as first-line therapy."

Olmesartan is also from the angiotensin II receptor antagonist classification also known as ARB. Therefore a complete cardiology assessment including echocardiography, urine proteins and detailed kidney function tests is a must before switching from ARB such as Olmesartan to ACE inhibitor.

List of ACE inhibitors include the following medicines
Enalapril
Ramipril
Quinapril
Perindopril
Lisinopril
Benazepril
Imidapril
Trandolapril
Cilazapril
Fosinopril

List of ARB medicines includes the following
Valsartan
Telmisartan
Losartan
Irbesartan
Irbesartan
Azilsartan
Olmesartan

As there are many medicines available, the right one should be given to you after detailed clinical examination and laboratory tests. The final option lies with your treating doctor depending on your long term follow up.

Your blood glucose is under moderate control as noted from HbA1c which is 7.5 percent.

Hope your query is answered.
Do write back if you have any doubts.
