Question: Dear Doctor,

1) Im worried about my 1 year old child. My neighbour lady in my apartment was feeding something to my 1 year old child through her finger into my babies mouth which happened unexpectedly. She has too many relationships and her husband also goes to prostitutes. So i doubt she might have any STD. Ladies will usually have cut in fingers because of nail biting and vegetable cutting. This i have noticed in my mother, sister fingers. Is there any risk for HIV, Hepatitis B, Hepatitis C, Syphilis, Chlamydia, Gonorrhea and Herpes through this exposure. Im worried about the cuts and scratches in the fingers.
2) I read in the internet about Mutated HBSAg in Hepatitis B infection. If it is present in blood it will produce false Negative HBSAg test results. If mutated HBSAg is present in a person and if it is transferred to another person, the person who received the transmission will also have Mutated HBSAg? or the person who only transmits will have mutated HBSAg?
3) Does Chlamydia and Gonorrhea are transfered into blood through cuts in the fingers during fingering Vagina?

I can understand your concern for the possibility of acquiring any Sexually transmitted infections after the incidence.

I have diligently analyzed your complete query and noted your concern. After thoroughly interpreting each point let me reassure you first that overall there are no chance of acquiring any Sexually transmitted infection in yiur child after getting feed from your neighbourhood person.

This is because of the fact that none of the major Sexually transmitted infection is transfered by simple feeding. In the transfer of any Sexually transmitted infection there should be mixing of blood or fluids from one person to another.

Even if we consider that the neighbourhood person have any STD then also there requires a minimum infective dose of bacteria or virus.

During feeding it is not possible to get that infective dose which is required to acquire the infection. If that could had been be case then every other person would have had been suffering from one or other STDs.

For mutated HBs Ag also it is not so much rampant that one could get it. Also it is not the common mode of getting the same by simple feeds.

The same is true for chlamydia and gonorrhea also. It requires long contact with enough of microorganisms to get the infection.

In my clinical experince simple feeding never results in any STDs.

Dear Doctor,

Thank you very much for your reply.

1) I understand that simple feeding will not transmit any STDs. If there is cut in her finger and that cut automatically becomes an open wound. When the lady puts her cut finger in my babies mouth automatically it becomes the open cut contacts my babies mouth mucosa. Is there any possibility of transfering infection when the cut contacts mouth mucosa or if there is any tiny unnoticeable cuts in my babies mouth and the cut of the lady contacts my babies cut?
2) If the lady puts her finger in her own mouth and immediately after that if she puts her finger in my babies mouth is there any STD transmission like Hepatitis b or HIV through Saliva?
3) Can you please tell me what will be the approximate infectious dose?

Below are General Questions i would like to know. Not related to my babies incident.
1) If mutated HBSAg is present in a person and if it is transferred to another person, the person who received the transmission will also have Mutated HBSAg? or the person who only transmits will have mutated HBSAg?
2) If a guy is fingering a woman in Vagina with cuts in fingers. Does Chlamydia or Gonorhhea will transmit to blood through cuts in the finger?
3) Drinking alcohol the night (8 hours) before going for STD testing will affect any of the STD testing?

Theoretically there are chances of getting HBsAg through cuts from one person to another. But practically and clinically this all depends upon the infectivity status of the person as well as the type of wounds in both the persons and the time period of contact of the infective material transferring from one person to another. So the infective dose will vary as per various situations described above.

HBsAg is more infectious than HIV. Practically I have not encountered any case getting hiv by simple feeding even being having cuts. In out mouth the salivary enzymes decreases the infectivity of HIV virus. So hiv is no transferred by saliva.

For HBsAg as most of the children in both developed and developing countries are giving vaccination for the same. So just check from yiur doctor if your child is vaccinated for the same or not.

If a person gets mutated HBsAg then will get the same type of mutated virus and will further transmit the same.

Gonorrhoea and chlamydia are basically transmitted through sexual transmission but the cut wounds through genital mucosa could also transfer the same. Though it is uncommon mode of transmission through cut wounds from sites other than genital mucosa.

Alcohol drinking does not affect the status of any tests related to Sexually transmitted diseases.

Dear Doctor,

Thanks for your reply and patience in answering my anxiety questions.

1) I understood that transmission of Hepatitis B through wounds depends upon the type of the wound, size, viral loads of persons transmitting and duration of contact.
In my babies incident the contact was maximum 5 seconds. Just only one finger she put in my babies mouth. If the lady has cut of 7mm and the depth of the wound was 2mm maximum. Consider the lady has high viral load. My feeling is the cut was not bleeding. In this situation what is the chance that my baby gets transmitted to mouth.
PLZ TELL ME IF THE LADY CUT HAS TO CONTACT MY BABIES CUT IN THE MOUTH OR EVEN TRANSMISSION IS POSSIBLE IN MOUTH WITHOUT CUT IN MY BABIES MOUTH.

2) As you said my baby is vaccinated for Hepatitis B. I read in the internet that if mutated HBSAg is present in transmitting person the transmission will occur even after Vaccination. Is it true?

3) Is the regular HBSAg kid test available in labs nowadays will find the mutated HBSAg? if not what is the reliable test to find hepatitis B infection.

In XXXXXXX screening for Hepatitis B is done based only on HBSAg.

In the above situation yiu have mentioned practically there are negligible chances of acquiring the infection by mouth. It becomes even less if the child is vaccinated.

It is true that mutant HBsAg can surpass the vaccination. But the cases of mutated viruses are less in Indians.

Routinely we for simple kit to detect HBsAg. But for any specific case is there then we go for the detection of any of the HBsAg by ELISA or PCR test. But these are not routinely done procedures and are performed only in specific cases.

I reassure you to not to take such a great stress for your kind of situation.

Dear Doctor,

Thanks for your reply. My last and final questions for you. Thank you for all your help.

These are the questions for my knowledge. Because the internet websites they confuse people by putting wrong informations mostly.

1) Does Saliva Transmits Hepatitis B?
2) Untreated Chlamydia and Gonorrhea will enter into blood? If true what is the blood test to find if it enters blood?
3) Does regular Urine Analysis Test will find if Chlamydia and Gonorrhea present? Here in XXXXXXX when one of my friend when he went to Sexual Disease Specialist to check for Chlamydia and Gonorrhea infection. He only did regular Urine Analysis test in the morning first urine. When the result came he said no infection present. When my friend asked him the doctor said PUS Cells were only 2-3/hpf so no infection present. But when i search in the internet they are saying we have to do NAAT Urine test for these infections.

Plz tell me can we just rely on Urine Analysis test if the PUS cells is 2-3/hpf?

The most common mode of infection of hepatitis B is by blood. However it could be transferred through saliva also.

Gonorrhoea and chlamydia in general never enters into blood but in certain immunocompromised persons it could. It sis only rarely in some individuals where the immunity is low.

In all the cases if chlamydia or gonorrhoea we prefer to go for urine examination in XXXXXXX But we can go for NAAT assay and PCR respectively. As the testing facilities in XXXXXXX are not so advanced so we basically go for urine test first and then follow higher tests.

Detection of 2 to 3 pus cells per hpf in urine examination is considered normal. It is usually detected in out routine tests. So the doctor is basically correct. But above and all I basically follow clinical findings and then correlate these with laboratory test of basic and advanced nature.
