Question: Hi Doctor, I had body to body massage as well as protected sex with a call girl on XXXXXXX 26 this year (2016) in Thailand.. Last few days, i noticed a painless bump (only one.. small one.. looks like pimple) on my scrotum.. there are no other symptoms so far. i am really scared.. can you please give me the list of tests to be done to check for any STDS or HIV? pls note that it's 42 days (from XXXXXXX 26) since I had sex last.

Hello,

There is nothing specific to say about symptoms of STD's in your description.
Since it was a high risk encounter you may get screening test for all STDS even though it is more than 40 days since the encounter. There is an standard list of tests available at any lab for STD's. You may go ahead with that. The list includes the following:

VDRL for Syphilis
IgG antibodies for Gonorrhoea
ELISA for HIV
Antibody for Hep B
Antibody for Hep C

1) I read that there are two types of hiv (1 & 2).. will elisa cover both?
i also read elisa will be useful only 3 months after the encounter as antibodies are produced only after months.. is that true for all antibodies test as well as elisa? is there a better test available to detect it before 3 months?

2) What about the tests for Herpes 1 and 2? again i read lgg mayn't be useful before 3 months and lgm mayn't be reliable.. is that the case? is there any test to detect it before 3 months?

3) What's your gut feeling on this issue? could I have std or herpes? i can send you the picture of the bump on my scrotum.. may be that will give some idea.. how to share the picture?

4) Could you pls give me the exact test names (as you write in prescription) so I can use this as prescription for the tests?

MAY NOT BE STD. YOU CAN UPLOAD A PICTURE OF THE SAME. ALSO LET ME KNOW IF YOU HAVE DISCHARGE FROM PENIS, BURNING URINE, BODY ACHES

ELISA FOR HIV COVERS BOTH. ELISA CAN BE TESTED FROM ONE MONTH OF EXPOSURE, the 90% antibodies shall form by then. At 3 months at least 95% form.

THE STD PANEL HAS ALL THE RELEVANT. HERPES ANTIBODIES ARE PRESENT IN MOST OF US EVEN WITHOUT SYMPTOMS THAT MEANS EVEN IN LOW RISK PEOPLE, SO YOU WILL HAVE IT AS WELL. TESTING AND THEIR INTERPRETATION IS A SPECIAL SKILL.

1) I did all the blood tests except TPHA and Chlamydia trachomatis IgG Antibody. I received the reports except for HSV. Please do review the reports and let me know whether I need to be concerned about something.

2) TPHA:
is this test for Syphilis? since vdrl (which i guess is for the same std) is non-reactive (which i believe means not positive), do i need to get this done?

3) Chlamydia test:
do i need this?

4) Azithromycin
After having that encounter on XXXXXXX 26, I took Azithromycin 500mg for 5 days,.. would that have cleared any bacterial infections such as Chlamydia or gonorohea I could have had?

is there any test to detect this? I heard that there's a vaccine for this. Could you please prescribe the exact vaccine for this and also the dosage and number of times I need to get this vaccine? also please let me know where it is available

except this painless bump, i don't have any other symptoms such as painful during urincation or pus discharge from penis, body pain, fever, etc.. but i have sore throat today and having cold for last few days..

ALL TESTS DONE WERE NORMAL AS EXPECTED.

NO IT IS FOR TREPONEMA. NO NEED TO RETEST. VDRL IS FOR SYPHILIS AND IS FOUND NEGATIVE

IT IS NOT STD IT LOOKS HAIR FOLLICLE INFECTION HEALING. IT HEALED WITH AZITHROMYCIN WHICH IS VERY STRONG ANTIBIOTIC

Thanks a lot for elaborate responses and advice. Appreciate that.

have couple of queries:
1) Hair follicle infection
Your diagnosis was excellent. When I saw pics of hair follicle infection, mine looks exactly same. also one clarification on azithromycin, i took this in XXXXXXX and not now. this infection I got it only in last few days..
is any medicine I need to take for this infection?

2) Herpes:
Am still single. in case i am diagnosed with Herpes, can I marry? or should I avoid marriage as there is a danger of me infecting my partner? also am not sure what kind of problems (like painful sores, etc) it can give when it gets activated.. don't want to suffer and don't want another person to suffer because of me

3) Tests:
you mentioned that I don't need further tests.. is it because i should have had some symptoms within this 40 days if at all i was infected of any stds?

4) Vaccines:
I am taking Hepatitis vaccine. i heard vaccines are available for typhoid and other diseases also.. could you prescribe those vaccines?

It was healing pictures. I checked all of them the last ones were healing. So do not need to take any more pics

2) Yes you can marry. As I said Herpes is there in all of us practically. It does not hinder the wedding. The transmission of Herpes is not known in some people they were out of marraige relationship.

3) It simple you did not acquire STD.

4) you can take vaccine for typhoid, hepatitis A, Tetanus.
