Having Scratching And Painful Swelling When Clapping Hands. Cure?

Question: For 6 months i have these symptoms: hives when scratching and slightly painful swelling when clapping hands. First appeared after i drove a brand new rental car & walked bare feet on hotel carpet. No other changes in e.g. food during that time.

I can understand your concern for the hives. It is really distressing for anyone to have anything unusual on skin.

It appears to be from your query that you have the problem of “Physical Urticaria” with "dermographism".

This is a common condition caused by bursting of allergy producing cells known as "mast cells". As the chemical inside them is released it produces a rash which you have mentioned. There can be multiple factors producing this condition which are included in day to day life and some times it is really difficult to find one exact cause.

In your case you are correlating it is due to the physical factors such as due to scratching, tight hoding of anything, pressure and een by tight clothings or rubbing against some surface. So the best treatment is to aoid such situations and it is better to stay away from the offending factors.

For the betterment you can apply over the counter calamine lotion 3-4 times a day. You can do cold water compresses to reduce the intensity of itching. Do not scratch or rub the skin as it can lead to further aggravation.

You can take oral anti histamine like loratadine or levocetrizine under medial advise.

Kindly don't get panicky about the situation as I told it is totally a simple and manageable condition. It never turns to any serious disease. If you have any fear of anything serious in your mind then kindly relieve the tension of the same.
Despite above measures if the problem is not getting better then it is better to consult a dermatologist for best guidance.

Thank you Dr. Kanodia, my concern was raised by the persistency of the condition, even when i returned to my home environment as well as the fact, that i never experienced such symptoms before. I do understand the cause of the hives or skin irritation, but not the swelling and increase of temperature of my hands when e.g. clapping. If i clap hard it gets even painful and the symptoms only disappear over the course of up to 15 min. The only thing that helps is putting my hands into the freezer box. I would really appreciate a short comment on that. Thanks again, XXXXXXX

Welcome again and thanks for your prompt reply.

Our human body is one of the best creations made by God. Despite this fact, all of us have some good or bad things going in our body every day, every minute and every second.
There are certain things which we always feel or experience for the first time. I can understand your concern that you have never had experienced such problem before, but to guide you, this problem is a simple process due release of certain chemicals under the surface of the skin. What most of us see it as a disease or hypersensitivity reaction..!!

But I guide my patients that this is a process where our immunity is bit increased resulting in more number of mast cells (a type of white blood cells). So due to this immune mediated process any time anything which is causing bursting of cell will result in the feature which you experience in day to day life. So it is on the other part a simple phenomenon which is telling everyday that your immunity is good..!!!

To get better of the condition we ask the patient to stay away form the aggravating conditions and to take antihistamine drugs. Though there are many types of antihistamine drugs available but in my consensus according to the clinical experience and scientific guidelines, I prefer to prescribe “loratadine” tablet by mouth.
If the condition is not controlled by one tablet then can be taken for two, three and up to four times a day. Once the symptoms are reducing, it can be tapered off slowly by making tablet one time then alternate days and then skipping for some more days in week.

As I said previously this is common condition which we see in our day to day practice and the most important part of the treatment is by patient education by explaining the facts as I explained you above. Kindly don't get panicky regarding the problem and it never turns to any serious disease and so relieve the tension of the same.
