Have had diabetes, done blood test. Findings?

Hi Doctor ,

My father aged 49 years was suffering from Diabetes from past 15 years, And yesterday for the first time in 15 yrs he went for the check up and i've attached the report here ..

Please check this report and tel me what can be done .

Thanks for your query.

I have reviewed your father's reports and coming to the following conclusions
He has uncontrolled diabetes, diabetic nephropathy, hyperlipidemia, fatty liver and mild anaemia.

He needs medical help to control the diseases and bring all the parameters to near normal. If untreated he is prone to develop complication like renal failure and coronary heart disease.

He needs a multidisciplinary team of doctor to manage this conditions.

In the first place consult a diabetologist and control this blood sugars.
This HbA1c is very high and at this level he needs insulin to control this blood sugars.
He is losing lot of proteins and little blood through urine due to Diabetic nephropathy.
To reduce the progression he needs
1. Tight control of blood sugar.
2. Protein and salt restriction in the diet.
3. Consultation with Nephrologist.

Hyperlipidemia needs to be managed with
1. Low fat diet(Avoid fried foods)
2. High fiber containing diets(vegetables and greens).
3. Regular exercise for 45 to 60 mins for 3 to 5 days in a week.
4. Medicines like statins.

Repeat all the investigation after 3 months and let me know the reports to see how your father improves.

Please help my dad with some diet plans to control the problem ..

And if possoble please suggest how much insulin is needed in a day.

He needs a Personal diet chart by a dietician.

Detailed Answer:

The requirement of insulin depends on body weight and blood sugar levels(FBS and PPBS. HbA1c).
It can be started as 2 units of insulin/kg of body weight/ day.
It is better you take the help of a doctor for a practical dose of insulin for your father.

Come to diet
1. Take whole grains(Avoid refined carbohydrates and sugar).
The total calorie requirement can be calculated by 25 kcal/kg/day.
2. Low fat diet(Avoid animal fat, full cream milk, butter , ghee and fried foods)
3. High fiber diet( Add vegetables and greens).
