Hello,XXXXX
Thank you for contacting XXXXXXX with your question. Just by
reading your symptoms , looks like the gas build up in the
intestine is what is causing the pain. You are twenty years old
and according to your reports , all the tests have turned out
normal. The PPI that you were taking is not helping you since
you do not have acid reflux problem.
What you have is "Irritable Bowel Syndrome" associated with
abdominal pain, distension and constipation . This is not
psychological. This is a Brain-Gut disorder mediated by hormone
Serotonin. People with IBS have higher sensitivity to gas bloat.
There are no effective medications for this. Mainly , you need
to be on a high fiber diet or take fiber supplements like
syllium powder daily. You have to drink enough liquids daily.
Regular exercises are helpful. If very constipated, you can try
taking stool softeners daily. You can also try a PEG solution,
polyethylene glycol, daily to regulate your bowels.
Peppermint oil capsules 1-2 daily can reduce the gas.
As for the small Hiatus hernia , I would not worry about that if
you are not having acid reflux symptoms such as heartburn,
recurrent sore throat or cough at nights.
If you are under a lot of stress, try to relax as much as possible
by listening to soothing music or relaxation CDs that are available.
You have a long and healthy life to look forward to.
I wish you the very best.

Hello,XXXXX
If you do get heartburn or reflux symptoms then you need
to be on a PPI daily once or twice a day. If the current drug
is not effective , your doctor can switch you to a different
PPI. Most of the time that works. It is also important that you
take the PPI about 60-90 minutes before your main meal of the
day and evening meal. Avoid tomato based products, citrus, excess
caffeine. alcohol , tobacco , very spicy , fatty, fried foods and dairy
like cheeses etc. Hope this helps.
Wish you well.
