Does My Lab Report Indicate Any Problem In Conceiving?

Question: Hi, I am 29 years old weighing about 80kgs with 5.3" height. I am married for 2.5 years now. I was conceived first time after 4 months of marriage. But I had to abort as I had taken lot of medicines due to a fall and my doctor advised me to take off. Second time again I conceived after 1.2 years of marriage but after 10 weeks I had miscarriage as baby didn’t had heartbeats. Now we are trying to conceive but in vain. I got my tests done just before the day I got my periods. LH=2.06, FSH=4.37, Prolactin=26.60 and AMH (Elisa) = 1.17 which is low fertility. Thyroid and blood sugar levels are normal. I am now taking melmet500 once a day and fertyl100mg from my third day of period and also vitalin tablets. Is it difficult for me to conceive naturally? What must I do now? Any treatment is required?Please help

Brief Answer: lose weight. increase melmet 500 mg thrice daily Detailed Answer: Dear XXXXXXX Thanks for writing to healthcare magic. Your history reveals that you don't have problems conceiving. You conceived twice in 2.5 years but last time you miscarried. Your blood tests are ok. AMH levels are used along with FSH, LH, and ultrasound to check for fertility potential. You FSH AND LH are in normal range. Prolactin is mildly high. What was your second day ultrasound report? Did it mention PCOS? Your weight is of concern. Women with obesity may miscarry. So please consult a dietician and plan to lose atleast 10-15 % of your present weight. You will see excellent results. Many patients conceive with just weight loss and tab fertyl. Also increase this melmet to 500 mg three times a day. Take tab ovacareforte daily. Continue vitalin. Follicular ultrasound to be done in this cycle from day 11 on alternate days. When your follicle is 20 mm your doctor will give you HCG inj. Intercourse has to be planned around the date of injection. I don't think you have low fertlity after examining your reports. Thank you doctor. Well my doctor did a scan on 7 th day of my cycle n said on one ovary folliculars are developing which is the result of fertyl100. Other ovary nothing could be seen. She didnt give me reports as such.melmet i am taking thrice a day now. She asked me to visit her on 11 th day of the cycle. She has given tablet for prolactin levels to come down. I am just hoping this month i can expect some good news

Brief Answer: response to fertyl is good. Detailed Answer: Dear XXXXXXX Thanks for writing back. Looks like the response to fertyl is good. Ideally one ovary should have one or 2 follicles growing. Your day 11 scan will tell us the chances in this cycle. By day 11 Your follicle should measure 14 mm. Endometrium or the uterus lining should also be thick. Please keep faith. The chances of pregnancy in each cycle even in healthy women is only 15-20%. Take ovacareforte. It's a vitamin meant for infertility. Also consider weight loss. Thanks Dr Bhagyashree
