Does Intake Of Truvada Prep Help In Preventing HIV?

Question: Hello
I got a question regarding Truvada PreP

This darn anxiety over getting infected is driving me crazy !

I have sex with lots of different women , weekly .
And every once in a while something happens where it scares the crap out of me .
Two weeks ago i noticed a fresh paper cut on my finger that i just used on fingering a woman .
I use condom for vaginal sex and no condom for oral.

2 weeks later ( now ) I have a flue , sore throat, cough, slight discomfort under my right chin towards lymph node. ( lymph node does not seem swollen )
Also i feel very tired most of the day, also I have stuffy nose .
No fever , no chills ,

I went to my doctor cause this flue started to get worst
He looked at me and said i have sinus infection
And gave me Amaxo clav 500 mg twice a day for 7 days.

Today is 2nd day im taking the drug , it seems i am starting to feel better , still not 100 %

But this anxiety is driving me crazy .
I am diagnosing myself already with ARS.

Does these symptoms sound like ars ?

Also i am considering to get preP . Truvada
I know they recommend you take itevery single day
Due to the coast of the drug i want to use it 3-4 times a week .

I read that if used every day it will protect you from hiv 99% and 4 times a day 96% protection .

So my question is do you know if i take truvada
One day before my encounter , then one pill on the day of the encounter and then third pill a day after ,
Does this sound like enough protection ?

Will one day on Truvada be enough to build up in my system to keep me somewhat safe ?

Brief Answer:
Pre Exposure Propphylaxis has to be taken consistently daily.

Detailed Answer:
Hi,
Welcome once again to HCM.
Thanks for posting your query.
Before taking Tiruvada (PrEP), rule out HIV with adequate HIV tests.
Tiruvada as a Pre Exposure prophylaxis has to be taken daily basis. The effectiveness of Tiruvada as a PrEP is not 100% safe. It reduces the chances of getting infection considerably (90-92%) if taken consistently (daily). Intermittent usage would not be able to protect you from HIV. Your risk for HIV does not get reduced by taking Tiruvada on 3-4 times a week. Capsule Amoxycillin twice daily for your Flu like syndrome is not adequate. It has to be taken 3-4 times daily.
Get well soon. Protected sex is safe also for oral sex.

You are saying i should be taking 4 doses of amxo clav per day .

Are you suspecting I have ARS?
Do people get treated with antibiotics for ARS ?

Brief Answer:
FLUlike symptoms are common.HIV testing is necessary to confirm or rule out

Detailed Answer:
Hi,
Welcome back.
If the symptoms are severe you have to take Amoxycillin 4 time a day, otherwise it is enough for you to take 3 times a day. (never 2 times)
Nowadays the term 'ARS' is not used. I think you are asked about acute infection (Primry HIV infection or Seroconversion illness). Capsule Amoxycillin have no role in the management of acute infection flu like manifestations. (ARS is different from acute infection and that term is not in use nowadays)
Antibiotics can influence bacterial infections and not viral infections.
Flu like symptoms, sore throat, lymph node enlargement and discomfort and weakness can occur with anybody even in the absence of HIV. Only by testing for HIV only one can either confirm or rule out HIV infection.

Brief Answer:
Amoxycillin will not be useful in HIV acute infection

Detailed Answer:
Hi,
Welcome back.
I can suspect that an individual would be likely to have acute infection. But I can fix the diagnosis of HIV acute infection only after the blood test reveal the same. As long as I am not sure about the diagnosis I will treat the throat infection as a streptococcal (bacterial) one and prescribe capsule Amoxycillin. After I came to the conclusion of HIV acute infection, then Amoxycillin becomes useless. When you got better with antibiotics, it was nothing but a bacterial infection and not due to HIV infection.

ohh thank god , You made me feel more at ease .

So, basically with Acute syndrome and flue like symptoms Antibiotics would not
make you feel any better. And a person is stuck feeling like crap until the acute
syndrome is done for 1-4 weeks.

I have made an appointment with my doc to talk to him about prep
I think it will give me extra piece of mind .

One last question doc.

I have seen some documentaries on HIV AIDS , and they say that
there are some people that are more or less immune to HIV
because genetically their t-cells do not have the little arm or maybe a protein that HIV needs to attach itself . Do you know if a person can check to see what kind of t-cells they have ? What would this test be called so I can ask my doctor ?

Brief Answer:
HIV Topism test will detect whether you have natural resistance to HIV

Detailed Answer:
Hi,
Welcome back.
Symptoms of acute infection are self-limiting and spontaneous recovery will lead to another longest asymtomatic period in case of HIV infection (before the manifestation of HIV symptoms), but would not respond to Amoxycillin.
Two types of receptors are essential for the entry of HIV into the T4 lymphocytes.
1. CD4 Receptors
2. CCR5 or CXCR4 co-receptors
1% Caucasian poppulation are lacking this CCR5 co receptors. In whom HIV can not gain entry and they are immune to HIV infection by nature.
There is a test available with bigger laboratories called as HIV Tropism test. That will reveal whether you are resistant to HIV or not.

Just got my results
This is test was taken approximately 14 days after my possible exposure.

This test was performed using the APTIMA(R) HIV-RNA
Qualitative Assay (Gen-Probe). Early Detection

It came back NEGATIVE

Do you feel 2 weeks would have been sufficient enough ?

Answered by Dr. Sankaranantham Murugan (2 hours later)
Brief Answer:
Negative HIV Qualitative RNA PCR is highly sensitive and reliable.

Detailed Answer:
Hi,
Welcome back.
Negative HIV Qualitative RNA PCR test by 14 days after the risky contact is highly reliable and very sensitive. But it can be considered conclusive by 4 weeks. More over HIV Qualitative RNA PCR is available separately for HIV 1 and 2 infection and not common both for HIV 1 AND 2 as that of antibody detection tests which can become positive only after the window period (6-12 weeks after the exposure).
HIV 1 infection is common to the extent of >99% throughout the world except in certain parts of XXXXXXX The prevalence of HIV 2 infection is less than 0.5%.
You are almost free from HIV 1 infection by this result.
