Question: I M suffering from bipolar disorder .sleep disorder is also there.Now my psychiatrist put me on gabapin 1200mg , quitipin 200 mg(plan) and librium 50 mg.May I take Dicorate 500mg librium25mg and quitipin 200 mg(plan) instead of those medications?

Thanks for writing back to us.

You want to omit gabapin and take less dose of librium. You can do that. But it may not be helpful for sufficient sleep as your lonazep is not there. I recommend taking librium 50 mg later on if you feel sleep problem. As such dicorate is preferable than gabapin as mood stabilizer. You need to take prescription of dicorate from doctor.

Inform these changes to your doctor also.

so may i continue to take librum 50 mg as it was taken from two days only.omition of gabapin may disturb my sleep? can dicorate 500 mg help me to have sufficient sleep sabilise my mood? Am I became tolerent to clonazepam as it was taken 2mg per night since 1998? yesterday I took 1200mg gabapin 50mg librium and 200mg quitapine(plain) which gave more sleep and i could awake at 9.A. M. today. Please guide what should I do?

Yes it is better to continue librium 50 mg as you want to stop gabapin. As stopping gabapin cold turkey may lead to some sleep disturbance. Librium can help in that situation.

Dicorate will help in stabilization of mood not in sleep. However you need approximately 1000 mg dose if your weight is more than 50 kg.

Yes after taking clonazepam for 17 years you can be tolerant to clonazepam. That is why I have recommended to increase the dose in previous communication.

If I were your treating doctor I would recommend reducing gabapin to 600 for 5 days and then stop. Add dicorate er 500 and after five days take it twice a day.
Even after this if you have more sleep, reduce librium later on.

So Sir i started Dicorate 500mg 200mg quitapine(plain) 50mg librium. I also continue 600mg gabapin for 5 days then i will stop it Please confirm my medications.i will start dicorate 500 mg twice a day as my weight is 70 kg. Not only that sir I am having very brilliant academic career and been working as Super class-1 officer in XXXXXXX govt. Sir its only for u to have any historical clue to my disorder.

Dicorate is for bipolar disorder.But sir so my experience is concerned i had to be treated for my sleep disorder/insomnia. so sir what should i do for fixing the remedy of my insomniac problem? omit ion of gabapin may disturb my sleep? sir is there any serious mental disorder from what i am suffering from ? Insomnia is a common symptom of bipolar disorder. Librium is benzodiazepine and in 50 mg dose it will likely treat your sleep problem completely. Gradually dose should be reduced later on as 50 mg is high dose. If you feel more sedation, reduce dose early. Simultaneously treating bipolar disorder will also treat insomnia automatically. Dicorate will do that work likely.

As I told gabapin is not used in bipolar and in insomnia it is used rarely. I have never used it in my life for sleep problem. I am not able to understand why it is prescribed in your case. It is mainly used in epilepsy and in neurological problems. From scientific point of view stopping gabapin may cause some sleep problem if you are using since long. But in your case with librium I guess no problem will be there.

Bipolar disorder as such is major disorder in psychiatry. However with current medicines person can live almost normal life. Regular check up by psychiatrist is recommended. When you feel any depressive or manic symptoms, consulting doctor soon would help in preventing XXXXXXX episode of depression or mania/hypomania. Maintenance phase is recommended lifetime after 3 episodes.

I know Dr Mrugesh Vaishnav very well. Yes many time dose prescribed by some doctors is high. Due to big name of doctor patient does not argue about medicine/dose or sometime doctor tells he knows more about drugs than patient. Hello,

Sorry for delayed response. If you have enjoyed sex means there is orgasm but no ejaculation then it is likely retrograde ejaculation. It means semen ejaculated reversely in bladder and it has likely came out with urine and passed unknowingly without your attention.
Quetiapine is likely culprit for this. Anti psychotic medicine often causes this side effect.
However considering age of 49 possibility of prostate and blood pressure medicine as cause should be ruled out.

- Do you have any problem in urinating?I recommend going for Serum PSA (prostate antigen testing) level as routine check up.

- Do you have blood pressure or diabetes? If yes any medicine is going for the same?

no sir ,there is no orgasm.i can not reach to it Even after 30 minutes. I recently GONE THROUGH routine check up and i am neither diabetic nor suffering from blood pressure. My protest is also found ok through sonography M i had enjoyed sex successfully before a week. i have no prostet because i regulararly used to aerobic exercise as well as PRANAYAM my lipid protocol is also ok. My PSA IS 0.672 NG/ML As per report dated 12/09/2015. i have been treating with Quitapine since last one year

As you have started dicorate and librium recently, librium can cause sexual dysfunction rarely. Anorgasmia is a rare side effect of librium and you are taking higher side dose. We can consider Librium as possible cause of your problem.

However this happens sometime without any reason. I suggest waiting for some time. Try sex again whenever possible without any apprehension. Just enjoy the process and do not think about orgasm and erection. You will likely succeed. If not we can stop the librium and check that you can do it or not to confirm the side effect.
