Question: Hello doctor
im a 22 years old male. i had protected vaginal sex with a female sex worker after having an oil body massage, the condom didn't leak or break as far as i noticed. 2 hours before the sex i shaved the area around the penis and my testicles and had some small cuts there from the razor, and i believe that her vaginal fluids got in contact with my testicles and groin area. 7 weeks after the sex i notice a rash on my buttcrack, the skin was very red there and it gets redder near the anus, no bumps or raises or pain or ichyness just red skin, and the anxiety started... 2 days after seeing the rash i started having some symptoms(due to either extreme anxiety or ARS or both) including fatigue, muscle aches,weakness, (high blood pressure and fast heart beat rate and chest pains, the anxiety probably), I didn't check for my tempreture for a fever... armpit pain (maybe lymph glands?)(i still have the armpit pain even 14 weeks after the sex) and groin pain then i went for an HIV ABS test at 7 weeks which came negative then i had an elisa 4th generation test and PCR test(either RNA or Proviral DNA) at week 8 which came negative, i got the PCR result 2 weeks after the symptoms started and i was very relived and symptoms went away ( the end of ARS or anxiety or both?) and i did a urine test which came with some bacteria so doctor gave me Neoxime 500mg antibiotics for 5 days 2 pills per day and some antiallergy pills and an antibiotic cream for the rash which didn't help i think but the rash went away on its own after about a month or more and the pain in testicles went away but now 14 weeks after the sex act they became red again.. my last HIV tests were done a 84 and 92 days after the sex( ELISA 4th generation ). so my questions are: 1)does the fact the i had an oil massage increase the risk of getting HIV if the prostitute was positive? 2) can i get HIV from the 2 hours old cuts on my testicles? 3)can ARS start at the 7th week after exposure? 3) do my symptoms resemble ARS( some to anxiety but some could be due to ARS?) 4)should i do more HIV antibody tests(at 6 months?) now or my tests are conclusive after 3 months? 5) have you ever seen a 3 months negative turning positive later? 6) is it true that only people with chemotherapy or IV drug users etc are the ones who take more time to seroconvert? 7)would the short course of antibiotics taken at around week 9 after exposure affect my HIV testing? 8) do I need to check for other STDs like gonorrhea for my red painful testicles?
sorry for asking many questions but my anxiety is killing me and the more i search online the more anxious i become. thanks in advance doctor.
1) i also forgot to include some symptoms which are diarrhea and bowel movments which also came at week 7 after discovering the rash which went away after getting the PCR test results, but later on after getting back to the mistake of googling HIV anxiety and fear came and diarrhea and lack of appetite came and weight loss also came back...

2.Since you had protected sex and the condoms did not slip or break , so in your scenario HIV transmission is very unlikely, but risk of Herpes remains a possibility.
3. No , Since your tests were negative including HIV RNA Test .
4.No the protocol is 3 months ( three tests-- at base line , 28 days and 90 days)

6.No chemotherapy or IV drugs have no bearing on HIV tests.

8.Yes you need to test for STD profile .

Your stomach upset may have different causes including anxiety and HIV can't be judged by symptoms. Anxiety itself can produce lack of appetite leading to weight loss. As you are aware now that these are general symptoms and in HIV too ,there can be general symptoms like fever , diarrhea , weight loss , eruptions etc . HIV can only be confirmed by blood tests which you did and all came negative as per protocol(@ or after 90 days)
Based on your information , I can say that acquisition of HIV in your scenario is very unlikely and you are HIV negative .

thank you for the answers doc but i'm wondering if that late ARS(at week7) would mean late seroconversion(beyond 3 months)?
and now i still have some pain in my armpits and i cant seem to locate any swollen lymph nodes and that pain is making it hard for me to forget about HIV.. what are your thoughts on this situation?

My answer to your question is 'no' To diagnose ARS , You need to have tested positive for HIV RNA test , which is not present in your case.
Your symptoms may be due to any other infections .
